网织红细胞血红蛋白含量在诊断育龄妇女隐性缺铁的临床应用

摘　　要：目的探讨网织红细胞血红蛋白含量(reticulocyte hemoglobin content,CHr)在判断女性隐性缺铁的应用价值。方法选取38例月经过多隐性缺铁(SF<14μg/l,Hb≥110 g/L)的女性作为病例组,68例健康女性作为正常对照组。CHr及血常规检测应用拜耳公司生产的ADVIA 120全自动血液分析仪及配套试剂。检测所有入选者的血清铁(SI)、转铁蛋白饱和度(TS)、铁蛋白(SF)等指标。结果隐性缺铁组的CHr明显低于正常对照组(P<0.01)。ROC曲线证实CHr比RBC、HGB、HCT、MCV、SI、TS、TIBC等指标能更好地诊断隐性缺铁。结论CHr是诊断育龄女性隐性缺铁的早期、敏感、特异的新指标。To investigate the usefulness of reticulocyte hemoglobin content （CHr） for diagnosing iron deficiency in premenopausal women. Methods 38 female patients with hidden iron-deficent and 68 healthy controls were selected. The levels of CHr and other hematological markers were determined by ADVIA 120 （Bayer diagnostics, Ireland）. Serum iron （SI）, transferrin saturation （TS）, total iron binding capacity （TIBC）, and ferritin （SF） were also measured. Hidden iron deficiency was defined as SF〈14 ng/mL,and HGB≥ll0 g/L. Results CHr indicated a high statistical difference between the hidden iron deficiency group and the control group （P〈0.01）. Receiver operator characteristic（ROC） curve analysis demonstrates that CHr has a better overall sensitivity and specificity than RBC, HGB, HCT,MCV,SI, TS,TIBC in the diagnosis of hidden iron deficiency. Conclusion Reticulocyte Hb content level was the better predictor of hidden iron deficiency in premenopausal women.
