左布比卡因和布比卡因硬膜外麻醉临床效果的比较

摘　　要：目的比较0.5%左布比卡因和0.5%布比卡因用于硬膜外麻醉的临床效果。方法择期在硬膜外麻醉行子宫全切手术的病人36例,随机分为两组,B组(n=18):0.5%布比卡因;L组(n=18):0.5%左布比卡因。选择T12～L1间隙行硬膜外穿刺,两组试验量均为2%利多卡因4mL,5min后根据试验量测得的麻醉平面,分别追加0.5%布比卡因和0.5%左布比卡因8～10mL,以麻醉平面达T9为宜。观察用药总量、感觉神经阻滞时间、运动神经阻滞时间、起效时间和副作用。结果L组感觉神经阻滞时间明显长于B组(P<0.05),而两组运动神经阻滞时间比较无显著性差异(P>0.05)。两组起效时间和用药总量比较无显著性差异(P>0.05)。结论左布比卡因麻醉效应和镇痛方面与布比卡因相似,但感觉阻滞时间更长。左布比卡因临床效果优与布比卡因。OBJECTIVE To compare 0. 5% Levobupivacaine and 0. 5% Bupivacaine in clinical effect for Epidural Obstruction. METHODS 36 cases of patients who were operated on total hysterectomy under epidural anesthesia were divided into two groups： Levobupivacaine group：0. 5 % Levobupivacaine （ n = 18 cases） ; Bupivacaine group. 0. 5% Bupivacaine （ n =18 cases）. Taking T12- L1 lines epidural puncture to be injected for 2% Lidocaine 4mL firstly, then to be injected for 0. 5% Levobupivacaine or 0. 5 % Bupivacalne 8-10mL,epidural block level doesn＇t exceed T9. To observe two groups of patients who use gross medicine dosage, feeling obstruction time, motoneurons obstruction time, effect of initial taking medicine and side effect. RESULTS The difference between two groups using gross medicine dosage,motoneurons obstruction time,and effect of initial taking medicine was not clear, the Levobupivacaine group feeling obstruction time was relatively longer than that of Bupivacaine group. CONCLUSION Anesthesia effect and anelgisia of Levobupivacaine was similar to that of Bupivacaine, but feeling obstruction time was longer. 0. 5% Levobupivacaine was better than 0. 5 % Bupivacaine in epidural anesthesia.
