摘 要:目的:探讨二维彩色多普勤超声(2D-CDUS)在宽颈动脉瘤动物模型介入治疗中的应用价值。方法:建立犬双侧颈动脉宽颈动脉瘤模型,并分别于治疗前、治疗后即刻、3个月和6个月对单纯支架植入组(Ⅰ)及支架植入结合电解可脱弹簧圈栓塞动脉瘤组(Ⅱ)进行2D-CDUS检查,观察动脉瘤的形态、大小、动脉瘤及载瘤动脉内部回声,定量分析各组治疗前后载瘤动脉及动脉瘤内血流参数,并与同期的血管造影结果相比较。结果:各组载瘤动脉治疗前后回声及血流动力学参数无明显改变。单纯支架植入组部分瘤体变小,回声无明显变化,内部血流信号逐渐减少,流速减慢(P<0.05);支架植入结合电解可脱弹簧圈栓塞组瘤体回声增强,内部血流信号明显减少至完全消失,血流速度显著降低(P<0.05),与血管造影结果是一致的。结论:在动物模型中,2D-CDUS可以为单纯支架植入及支架植入结合电解可脱弹簧圈栓塞治疗颈动脉宽颈动脉瘤的疗效评价提供可靠的依据。To evaluate the application of two-dimentional color Doppler ultrasound (2D-CDUS) to interventional treating wide-necked aneurysm. Methods. A canine model of the wide-necked aneurysm was established, treated by intravascular stent and stent with Guglielmi detechable coil (GDC) respectively. Ultrasonography was employed to detect the aneurysm immediately, 3 and 6 months after the intervention. The size and ehco of the aneurysms and parent artery were observed by two-dimentional ultrasound. The parameters of hemodynamics of aneurysm and parent artery were quantitatively analyzed pre-and post-operation as well. Results: No significant differences were found in the ehco and blood flow parameters of all parent artery preand post-operatively. Compared with preoperation, the size of some aneurysm in group Ⅰ was smaller and the blood flow slower (P 〈 0.05). However the blood flow could still be detected 6 months after operation. In group Ⅱ, the echo of aneurysms enhanced, the blood flow intra-aneurysm decreased significantly(P 〈 0.05) and completely disappeared several months after operation. Conclusion: 2D-CDUS can accurately and conveniently describe the characteristics of aneurysm in the animal model, which is an effective way in evaluating experimental wide-necked aneurysm.
