医用植入体逆行交锁髓内钉治疗胫骨下端严重复杂性骨折并骨不连的特征与操作要点

摘　　要：观察逆行交锁髓内钉的植入物特点与治疗胫骨下端严重粉碎性复杂性骨折并骨不连、骨缺损的临床操作的关系,采用此方法治疗2000-04/2005-12解放军第一七五医院骨科收治的胫骨下端粉碎性骨折并骨不连伴有跟腱或腓总神经不可修复性损伤、骨折区有长节段骨缺损等复杂性骨折患者11例,均为GustiloⅢ度开放性骨折。11例中随访半年1例,1年4例,2年3例,3年2例,4年1例,所有患者骨折均获得愈合,平均骨折愈合时间为6.2个月。无一例出现内固定物断裂,无一例出现伤口感染,有1例出现跟下痛。结果显示医用植入体逆行交锁髓内钉治疗胫骨下端严重粉碎性复杂性骨折,固定可靠,术后可早期负重行走,能促进骨折愈合。To explore the characteristics of retrograde intramedullary interlocking nail and the relationship with the clinical treatment of severe comminuted and complicated fractures combined with nonunion and bone defect of the distal tibia. From April 2000 to December 2005, we used this method to treat 11 cases of severe comminuted and complicated fractures of the distal tibia, combined with nonunion and unprepared injury in achilles tendon.or common peroneal nerve as well as long-segment bone defect, which were open fractures of Gustilo type Ⅲ. of the 11 patients, 1 patient was followed up for 6 months, 4 patients for 1 year, 3 patients for 2 years, 2 patients for 3 years and 1 patient for 4 years. All fractures were healed, and the mean union time was 6.2 months. There was no postoperative infection and internal fixation break, 1 case got heel pain. The result shows that retrograde intramedullary supracondylar nail for severe comminuted and complicated fractures provides reliable fixation, and the patients can load eady after operation and it promotes fractures healing.
