摘 要:目的:观察胰岛素样生长因子Ⅰ在正常与突出椎间盘中的含量并分析其与病程、患者年龄的关系。方法:于2003-08/2005-01在安徽省立医院骨科收集腰椎间盘标本50例,自椎间盘突出症患者术中取出椎间盘组织,包括纤维环和髓核(L3~S1),分为椎间盘退行性变组和正常对照组。椎间盘退行性变组42例,年龄:≤30岁,12例;31~50岁,16例;>50岁,14例。病程:3~6个月,16例;7~12个月,15例;>12个月,11例。正常对照组,8例,年龄19~31岁。所有标本用苏木精-伊红染色观察椎间盘组织有无血管组织,软骨细胞是否增生和退行性变,纤维板状结构有无破坏,基质是否有增生,用来确定椎间盘软骨细胞是否有退行性变。采用胰岛素样生长因子Ⅰ的免疫组化染色,进行免疫图像分析。观察胰岛素样生长因子Ⅰ表达情况及其在不同病程、年龄患者中的阳性表达比较。结果:实验标本50例全部进入结果分析。①正常对照组软骨细胞呈纺锤形,位于软骨陷窝内,胞膜完整,胞质均匀,胞质内少见空泡,基质少见。椎间盘退行性变组软骨细胞增生,胞膜不清,胞质内有较多空泡,部分细胞核消失,细胞外基质增多。②所有标本胰岛素样生长因子Ⅰ均有阳性表达,阳性表达主要位于椎间盘软骨细胞的胞核,呈棕黄色。③椎间盘退行性变组≤30,31~50,>51岁3个年龄段的胰岛素样生长因子Ⅰ含量均显著低于正常对照组[(0.031±0.009)%,(0.029±0.009)%,(0.029±0.010)%,(0.045±0.007)%,P<0.01~0.05],退行性变的腰椎间盘中的胰岛素样生长因子Ⅰ含量在3个年龄段中差异无显著性意义(P>0.05)。④椎间盘退行性变组病程3~6,7~12,≥12个月3个时间段,随着病程延长,胰岛素样生长因子Ⅰ含量有明显减少趋势[(0.037±0.008)%,(0.029±0.007)%,(0.020±0.003)%,P<0.05]。结论:退行性变与正常的腰椎间盘中均有胰岛素样生长因子Ⅰ,但在退行性变腰椎间盘中含量较低。�AIM: To investigate the expressions of insulin-like growth factor Ⅰ (IGF- Ⅰ ) in normal and herniated discs, and analyze its relationship with the course of illness and the patients' age. METHODS: Fifty herniated intervertebral disc specimens were obtained from the Department of Orthopedics of Anhui Provincial Hospital from October 2003 to January 2005, including the fibrous annular and nucleus pulposus (L3-S1), which were divided into the intervertebral disc degeneration group and the normal control group. Of 42 patients in the intervertebral disc degeneration group, 12 were younger than 30 years old, 16 aged between 31 and 50 years, 14 were older than 50 years, 16 had been suffered from the disease for 3-6 months, 16 for 7-12 months, and 11 for more than 12 months. Eight subjects in the normal control group aged between 19 and 31 years. All specimens were examined by HE staining to study whether there were vascular tissues in the intervertebral disc, whether there were proliferation and degeneration of cartilage cells, whether there were destruction in the structure of fiber board and whether there was proliferation in the matrix, so as to make it clear that whether there were degeneration in the cartilage cells of intervertebral disc. The immunohistochemistry of IGF- Ⅰ as well as immune imaging analysis were performed to study the expression of IGF-Ⅰ as well as compare its positive expressions in patients with different course of disease and age. RESULTS: A total of 50 specimens were involved in the analysis of results. (1) The cartilage cells located inside the cartilage bone lacuna in the normal control group were in fusiform with complete cell membrane, even cytoplasm, less vacuoles in the cytoplasm and matrix. The cartilage cells in the intervertebral disc degeneration group proliferated with unclear cell membrane and much vacuoles in the cytoplasm, and partial nucleus disappeared with extracellular matrix increased.
