摘  要:目的:科学的体育锻炼对人的骨密度有积极的影响,综述运动对骨矿生化成分、骨形成生化标志物及骨吸收生化标志物的影响,以期为开展健骨运动处方的研究、探讨运动对骨质疏松的防治机制提供研究思路。资料来源:应用计算机检索Medline数据库1979-01/2003-12期间的相关文章,检索词“Exerciseortraining,boneormetabolism”,限定文章语言种类为English。同时计算机检索中国期刊全文数据库1990-01/2003-12期间的相关文章,检索词“运动或训练,骨或代谢”,限定文章语言种类为中文。资料选择:对资料进行初审,选取与运动对骨影响有关的文章,纳入标准:随机临床实验及基础研究,有对照组等。排除标准:重复研究或综述类文献。资料提炼:共收集到40篇关于运动对骨代谢或骨有关的实验或综述文章,选用24篇,分别为不同强度运动对骨矿生化成分、钙调节激素、骨形成和骨吸收指标的影响。排除16篇为重复研究或综述类文献。资料综合:①运动对骨矿生化成分的影响:血钙水平与运动负荷呈正比,持续运动后24h恢复阶段内,血钙增加,在两个持续运动之间的休息阶段钙离子和甲状旁腺素的变化削弱。②运动对钙调节激素的作用:钙磷代谢和骨再建过程主要受甲状旁腺激素、1α,25(OH)2D3及降钙素的调节。在最大无氧运动后即刻甲状旁腺素的分泌受到短暂的抑制,恢复阶段中又被刺激,交感神经的兴奋可能会引起甲状旁腺素水平的提高;甲状旁腺素浓度在运动中增加。③运动对骨形成指标的影响:体育运动对骨代谢的影响作用要依赖于运动的性质而定。游泳运动可以使血清骨钙素、B型血清碱性磷酸酶和Ⅰ型前胶原羧基端前肽等骨形成生化标志物显著升高,而骨吸收生化标志物I型胶原交联N末端肽显著下降。④运动对骨吸收生化指标的影响:通过对血浆抗酒石酸酸性磷酸酶活性在运动中OBJECTIVE: Scientific physical exercise has positive influences on human bone mineral density. In this paper, we review the effect of exercise on the bone mineral biochemical composition, biochemical marker of bone formation and bone absorption, so as to provide a research thought for carrying out the study of exercise prescription for keeping bone fit and for the prevention mechanism of exercise training from osteoporosis. DATA SOURCES: A computer-based online search of Medline database was undertaken to identify the related articles published in English from January 1979 to December 2003 with the key words of "Exercise or training, bone or metabolism". Meanwhile, we searched CNKI for related Chinese articles published between January 1990 and December 2003 with the same key words in Chinese. STUDY SELECTION: The articles were selected primarily to choose the articles about the effect of exercise on bone. Inclusive criteria: randomized clinical trails and basic research involving control group. Exclusive criteria: repetitive research or review articles. DATA EXTRACTION: Totally 40 articles related to the effect of exercise to bone or bone metabolism were collected and 24 met the criteria involving the effect of exercise at different intensities on the bone mineral biochemical composition, androgen of regulated calcium, indexes of bone formation and bone absorption. Sixteen repetitive researches or review articles were excluded. DATA SYNTHESIS: (1)Effect of exercise on the bone mineral biochemical composition: Blood calcium level is positively related with exercise load; during the recovery phase after 24 hours continuous exercise, the blood calcium level is increased, while the changes in calcium ion and parathyrin are not obvious in the rest stage between two exercises. (2) Effect of exercise on the androgen of regulated calcium: Calcium and phosphate metabolism and bone reconstruction are regulated by parathytin, 1α, 25(OH)2D3 and calciUonin.
