摘  要:目的探讨定量组织速度成像(QTVI)和核素心血池显像法评价高血压患者右心室收缩及舒张功能的应用价值。方法42例原发性高血压患者,在QTVI超声心动图检查结束后24 h内行核素心血池显像检查。QTVI测量右心室侧壁三尖瓣环处收缩期最大位移(DS)、收缩期峰值速度(Sm)、快速充盈期峰值速度(Em)、心房收缩期峰值速度(Am),计算Em/Am值。^99mTc-RBC平衡法核素心血池显像测定右心室射血分数(RVEF)和右心室高峰充盈率(RPFR)。并对两种方法检测量值进行比较。结果高血压患者DS和Sm与RVEF显著正相关,分别为:r=0.61,P<0.05; r=0.52,P<0.05。Em及Em/Am值与RPFR亦具有显著相关性,分别为:r=0.48,P<0.05; r=0.43,P<0.05。结论与核素法比较,QTVI法Sm、DS、Em及Em/Am值能够较准确评价高血压患者右心室收缩及舒张功能。Objective To investigate routine functional indexes of right heart ventricle by both quantitative tissue velocity imaging ( QTVI ) and radionuclide ventriculography in essential hypertension patients. Methods Forty-two cases with hypertension underwent radionuclide ventriculography 24 hours after QTVI echocardiography. The peak velocities of right ventricle(RV) lateral wall sites at tricuspid annulus were detected during systole, early and late diastole(Sm, Em, Am). The Em/Am ratios (Em/Am) and the peak systolic displacement (Ds) parameters were calculated. RV ejective fraction (EF) and RV peak filling rate (PFR) were detected by 99mTC-RBC gated control heart blood imaging in all patients. Results Sm and DS of RV lateral wall at tricuspid annulus were correlated significantly with RVEF measured by radionuclide angiography (r=0.61 and r=0.52, P〈0.05 respectively). In diastolic parameters, Em and Em/Am correlated with RV PFR (r 0.48 and r 0.43 P〈0.05 respectively), too. Conclusions QTVI-derived DS, Sm and Em, Era/Am of RV are accurate indexes to dedect RV dysfunction in essential hypertension patients.
