北京地区开展临床路径应用研究概述

摘　　要：概述了临床路径的概念、历史和国内外应用情况，介绍了北京地区开展临床路径应用研究的5个步骤：病种选择、表单制定、专家论证、小样本实验、总结修改和评审完善。结果显示，临床路径的实施能够比较好地规范医疗行为，缩短平均住院日，降低医疗费用，并能保证医疗质量，提高病人满意度。进而讨论了实施临床路径与按单病种付费的关系，临床路径与按诊断相关分类法（DRGs）付费的关系。以及当前实施临床路径应着重改善的客观环境。The paper describes the concept and history of clinical pathway as well as its application at home and abroad. Then the five steps in conducting the research on clinical pathway were introduced, disease choice, form design, expert verification, small sample trial, summary and revision, assessment and improvement. Results showed that the implementation of clinical pathway can regulate the medical behavior, shorten the average hospital stay, reduce the medical expenses, ensure the medical quality, and improve the patient satisfaction. The paper further discusses the relationship between clinical pathway and single disease charge as well as between clinical pathway and charge according to Diagnosis Related Groups （DRGs）, and indicates that the current environment should be improved for the implementation of clinical pathway.
