摘  要:目的研究儿童青少年腰围(WC)、体质指数(BMI)预测胰岛素抵抗(IR)或代谢综合征(MS)的意义。方法对超重33例、肥胖106例、正常对照(NC)儿童青少年140例进行身高、体重、腰围、Tanner分期、血压及OGTT、血脂、胰岛素的测定。结果总体人群中,BMI、WC与MS组分存在单因素相关关系(P<0.01),其中与HOMA-IR相关性最好(r=0.717,0.709)。多元线性回归分析显示,正常对照组WC(βcoefficient=2.169)和TG(βcoefficient=0.429)可预测HOMA-IR,而超重肥胖组BMI(βcoefficient=0.356)和TG(βcoefficient=0.886)可预测HOMA-IR。结论超重、肥胖儿童青少年的BMI可能是确定MS高危人群的指标。Objective To determine the association of waist circumference (WC) and body mass index (BMI) with metabolic syndrome (MS) in children and adolescents. Methods The 140 obese, 33 overweight and 106 nonobese children and adolescents took anthropometric measurements, including BMI, WC, blood pressure, and Tanner staging. The OGTT, lipid profile, and insulin assays were performed. Results In total population, there were positive correlations (all P〈0. 01) of WC and BMI with systolic blood pressure (r=0. 596,0. 586), diastolic blood pressure(r=0. 567,0. 557), triglyceride(r=0. 444,0. 438), cholesterol(r=0. 170,0. 170), fasting blood glucose(r=0. 152,0. 147) and HOMA-IR(r=0. 709,0. 717) respectively, and were negative correlations of WC and BMI with high-density lipoprotein cholesterol (r=- 0. 340,-0. 363) respectively. Multiple linear regression analysis indicated that in control group, WC (β coefficient= 2. 169) and triglyceride(β coefficient= 0. 429) were predictors of HOMA-IR, but in overweight and obese groups, BMI (β coefficient= 0. 356) and triglyceride(β coefficient=0. 886) were predictors of HOMA-IR. Conclusions BMI may be a useful tool to determine overweight/obese children and adolescents at high risk for MS.
