心脏和心肺移植术后彩超观察排斥反应心包积液变化的研究（附4例报道）

摘　　要：目的彩色多普勒超声心动图（UCG）观察原位心肺联合移植（OHLT）、心脏移植（OHT）术后急性排异期、稳定期心包积液的变化,评价心包积液作为排异指标的临床意义和价值。方法UCG对4例OHLT、OHT患者术后进行最长达五年余不等的长期动态监测急性排异期、稳定期心包积液的变化。结果本组病例发生十几次急性排异反应,出现中到大量心包积液,从未使用过心内膜心肌活检,均由UCG定性、定量观察、评判,明确提出超声诊断。结论UCG是测定心包积液量变化的重要方法,可作为评价OHLT、OHT急性排异期、稳定期反应的重要指标。Objective To observe pericardial effusion of orthotopic heart transplantation （OHT） and orthotopic heart - lung transplantation（OHLT） with UCG, and evaluate the clinical value of pericardial effusion as marker of acute rejection of OHLT and OHT. Methads Pericardial effusion was observed with UCG in 40HLT or OHT patients 2 days to 5 years. Results The increase of pericardial effusion is a characteristics of acute rejection. Without heart membrane cardiac muscle biopsy, UCG make a diagnosis and give treatment in time. Conclusions UCG is a very important tool to evaluate acute rejection of OHLT and OHT. Pericardial effusion is an important index of acute heart rejection.
