踝关节韧带损伤的早期诊治

摘　　要：目的:探讨踝关节韧带损伤早期诊治的重要性。方法:48例踝关节韧带损伤患者,男28例,女20例;年龄13~52岁,平均29岁。其中踝关节外侧扭伤46例,内侧2例。受伤至治疗时间3 h^2周,平均3.5 d。根据坎贝尔手术学分类标准:Ⅰ度损伤21例,Ⅱ度损伤18例,Ⅲ度损伤9例。48例患者均采用踝关节的固定制动治疗,支具固定27例,石膏固定12例,绷带绑缚9例。6周后拆除外固定。随访观察,对结果进行疗效分析。结果:48例患者均获得随访,随访时间10个月~4年,平均2.9年。踝关节功能活动良好,未出现踝关节不稳及功能活动异常。按照美国足踝外科学会制定踝后足评分标准进行疗效评定:优42例,良5例,中等1例。结论:踝关节韧带损伤应该引起重视,力争做到早期诊断,并早期行踝关节固定制动,以免引起踝关节不稳。Objective：To explore the significance of early diagnosis and treatment for ankle wrick. Methods： Forty-eight patients with ankle ligament wricked were studied including 28 male and 20 female, age 13 to 52 years （average,29 years）. The outboard wrick of ankle joint was occurred in 46 patients, inboard in 2 patients. The time from wricked to treatment were 3 hours to 2 weeks （average,3.5 days）, According Campbell＇s Operative Orthopaedics clinical symptom system, 21 patients were of degree Ⅰ, 18 patients of degree Ⅱ, 9 patients of degree Ⅲ. Forty-eight patients were fixed, 27 patients with ankle joint brace, 12 patients with plaster cast, 9 patients with bandage. After 6 weeks backout the fixation,to study the effect of ankle joint function. Results： The 48 patients were followed up 10 months to 4 years （average 2.9 years）, The ankle movement were all right, without instability and impedient, Based on AOFAS Ankle Hindfoot Scale, 42 patients were excellent, 5 patients were good, 1 patient was fair.
