摘　　要：目的:探讨糖脂代谢紊乱对女性代谢综合征(MS)患者左室结构与功能的影响。方法:将603例女性MS患者按年龄分为<50岁、≥50岁2组,每组再分为3个亚组:血脂紊乱[高三酰甘油(TG)和(或)低高密度脂蛋白胆固醇(HDL-C)]亚组、高血糖亚组及血脂紊乱合并高血糖亚组,对3组组间一般情况和左室结构与功能的指标进行分析。结果:≥50岁的女性MS患者舒张期左室前后径(LVD)、室间隔厚度(IVS)、左室重量指数(LVMI)、总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)均显著高于<50岁者(P<0.05),而E/A和舒张压(DBP)显著低于<50岁者(P<0.05)。≥50岁的女性MS患者血脂紊乱亚组的左房前后径(LAD)、左室肥厚(LVH)检出率及LVMI显著低于高血糖亚组和血脂紊乱合并高血糖亚组(P<0.05)。结论:≥50岁合并高血压的女性MS患者,高血糖对左室结构影响较血脂紊乱更显著,而对左室功能的影响无显著差异。Objective To explore the influence of hyperglycemia and dyslipidemia on left ventricular structure and function in women with metabolic syndrome （MS）. Methods Six hundred and three women with MS were divided into two groups according to their age 〈50 and ≥50 years, which were respectively redivided into three subgroups： dyslipidemia group [high triglyceride （TG） and/or low high density lipoprutein cholesterol （HDL-C）], hyperglycemia group and dyslipidemia combined with hyperglycemia group. Baseline states and indexes reflecting left ventricular structure and function were examined. Results Compared with the women〈50 years old, those≥50 years old had higher left ventricular dimension （LVD）, interventricular septal thickness（IVS）, left ventricular mass index（LVMI）, total cholesterul（TC） and low density lipoprutein cholesterul（LDL-C）, and lower E/A and DBP （P〈0.05）. A greater left atrium dimension （LAD）, prevalence of LVH and LVMI were found in the hyperglycemia group and the dyslipidemia combined with hyperglycemia group but not in the dyslipidemia group （P〈0.05）. Conclusions Hyperglycemia has much more significant impact on left ventricular structure than dyslipidemia in the hypertensive women with MS, whereas left ventricular function has no difference between the two groups.
