直肠钡剂排放技术在结肠双对比钡灌肠检查中的应用效果

摘　　要：目的提高结肠双对比钡灌肠检查(DCBE)中直肠-乙状结肠部位的双对比效果。方法对研究组连续267例患者,均采用我科设计的一次性结肠双对比检查用塑料袋及直肠钡剂排放技术进行DCBE。同时,随机抽取我们以往没有采用直肠钡剂排放技术的DCBE 300例作为对照研究。结果两组病例在直肠-乙状结肠区的双对比效果差异有统计学意义(P<0.01):(1)直肠钡剂排放技术能有效地减少直肠-乙状结肠内的过多积钡;直肠和乙状结肠内有过多积钡的病例,研究组分别占4%和19%,对照组分别占65%和44%。(2)双对比造影X线片优良率,研究组平均为87%,明显高于对照组的38%。结论在DCBE中采用直肠钡剂排放技术可显著提高直肠-乙状结肠部位的双对比效果,从而有利于该部位小病灶的检出及诊断。Objective To improve the quality of double-contrast image of the rectum and sigmoid colon in double-contrast barium enema examination of the colon（DEBC）. Methods Two hundred and sixty seven consecutive patients of the study group were examined by using self-designed disposable plastic bag and the rectum barium suspension drainage technique in DEBC. In addition, the radiographs from random sampling of 300 DEBC cases without using the rectum barium suspension drainage technique in the past were used as a contrast group and compared with the study. Results The results from the two groups showed significant difference in the quality of double-contrast image of rectosigmoid colon（P〈0.01）： （1）The drainage technique could effectively diminish excess residual barium pool of rectosigmoid area：the percentage of the cases with excess residual barium pool in the rectum and sigmoid were 4% and 19% respectively in the study group,while in the contrast group, which were 65% and 44% respectively. （2）Excellent radiograph rate of the study group was significantly higher than that of the contrast group. The average rates were 87% and 38% respectively. Conclusion Using the rectum barium suspension drainage technique in DCBE can significantly improve the quality of double-contrast image of rectosigmoid colon,and is beneficial to detect and diagnose small lesions of rectosigmoid colon.
