摘  要:目的探讨迷走神经刺激对实验性关节炎(EA)大鼠T细胞活化的调节作用及其机制。方法建立大鼠EA模型,将模型大鼠分为迷走神经刺激(VNS)组和假手术组。VNS组在完全清醒状态下予迷走神经电刺激,刺激参数:波宽1.0ms、电流强度3.0mA、频率16Hz、串长10s、串隔1.5min,每日30min,共4周。用流式细胞及免疫荧光技术检测大鼠外周血CD4+ T淋巴细胞活化(表达CD71)及表达烟碱样乙酰胆碱受体α7亚单位(nAChRα7)和胆碱乙酰转移酶(ChAT)的状态。结果治疗1周后VNS组大鼠外周血CD4+T细胞表达nAChRα7、ChAT明显高于假手术组(Pa<0.01);2周后VNS组大鼠外周血活化的CD4+ T淋巴细胞明显低于假手术组(P<0.01);治疗3周后VNS组大鼠关节炎指数明显低于假手术组(P<0.01)。结论电刺激迷走神经可以上调nAChRα7在CD4+ T淋巴细胞中的表达,激活免疫系统的固有胆碱能系统,通过抑制免疫细胞异常活化,减少炎性细胞因子合成及分泌,减轻关节炎症,从而实现对EA的治疗作用。Objective To explore the effects of stimulating vagus nerve with pulse current on peripheral blood CD4^+ T lymphocyte of rats with collagen induced arthritis and its mechanism. Methods To duplicate model rats of experimental arthritis(EA) by intradermal injection of Ⅱ type collagen, divide the rats into 2 groups: vagus nerve stimulation (VNS) group and sham operated group. Rats in VNS group were stimulated at the left cervical vagus nerves for 30 minutes a day with constant square wave, pulse current with intra train of 16 Hz, pulse duration of 1.0 ms, train duration of 10 s, interstimulus interval of 1.5 rain and intensities of 3.0 mA. Then flow cytometry and immunofluoreseence methods were used to detect the activation of CD4^+ T lymphocytes ( expressing CD71 ) and the expression of nicotinic acetylcholine receptors alpha 7 ( nAChRα7 ) and choline acetyltransferase (CHAT) in peripheral blood CD4^+ T lymphocytes. Results In VNS group, the expression of nAChRα7 and ChAT were significantly raised in CD4^+ T cells at 1 ^st weekend (Pa 〈 0.01 ) ;CD71 ^+/CD4^+ T cells in peripheral blood of EA rats were reduced significantly at the 2^nd weekend(P 〈0.01 ) ;the arthritis index of EA rats were stepped down markedly at the ^3rd weekend(P 〈0.01 ). Conclusion Vagus nerve stimulation can up - regulate the expression of nAChRα7 at the surface of CD4^+T lymphocytes, activate the cholinergic system in immune system, which perform a local and constant immune regulation to reduce the inflammation at last.
