中药复苏饮对严重脓毒症患者外周血单核细胞TLR2、TLR4基因表达的影响

摘　　要：目的观察中药复苏饮对严重脓毒症患者外周血单核细胞TLR2、TLR4基因表达的影响。方法:将20例患者随机分为复苏饮组(F组)和安慰剂组(A组),两组均采用西医常规监护治疗,F组配合使用中药复苏饮,而A组则加用安慰剂。另选取15名健康者作为正常对照组。采用RT-PCR法测定所有患者治疗前、治疗第3天、第7天外周血单核细胞TLR2、TLR4mRNA表达的变化。结果:治疗前两组患者单核细胞TLR2、TLR4mRNA表达显著低于正常对照组(P<0.05);F组治疗后第3天TLR2、TLR4mRNA表达即明显回升,且治疗后各个观察点F组的基因表达均明显高于A组(P<0.05)。结论:中药复苏饮可通过明显上调外周血单核细胞TLR4、TLR2 mRNA的表达,降低严重脓毒症患者的病死率。Objective：To observed the effect of Fusu yin capsule on the expressionof Toll - like receptor - 2 and 4 mRNA in peripheral blood monocytes of severe sepsis patients. Methods：20 cases in this study were randomly divided into group F and group A. Patients in group F were treated with Fusu yin capsule and routine western therapy, however cases in group A were treated with placebo and routine westem therapy. Another group of 15 healthy individuals were also included in the study as controls, on admission（before treatment）, on Days 3,7 after treatment, RT-PCR was used to detect the expressive change of TLR2 and TLR4 mRNA. Results：on admission, the expression of TLR2 and TLR4 mRNA were obviously lower in SSthan those in normal（ P〈 0.0 1 ）, however the expression of TLR2 and TLR4 mRNA increased rapidly in group F on day3 after treatment（P〈 0.05）：And on day3, dayT, the expression of TLR2 and TLR4 mRNA in group F were significantly higher than those in group A（P〈 0.05）. Conclusions：Fusu Fin capsule had the effect on decreasingme mortality of severe sepsis patients by upregulating the expression of TLR2 and TLR4 mRNA in peripheral blood monocytes.
