高渗盐水合用大剂量利尿剂治疗顽固性心力衰竭并低钠血症的临床观察

摘　　要：目的探讨高渗盐水合用大剂量利尿剂治疗顽固性心力衰竭并低钠血症的方法、安全性及有效性。方法将12例顽固性心力衰竭合并低钠血症患者,随机分为治疗组、对照组各6例。在常规治疗基础上,治疗组用3%高渗盐水治疗,在使用盐水治疗同时静滴硝普钠,使用盐水后静滴速尿100 mg。结果治疗组与对照组治疗前、治疗后1周血钠浓度分别为120.6 mmol/L±7.2 mmol/L和121.5 mmol/L±6.9 mmol/L、134.7 mmol/L±4.8 mmol/L和126.2 mmol/L±7.1 mmol/L(P<0.001);两组血钠改善时间、心功能改善时间和水肿消退时间分别为5.8 d±2.6 d和10.1 d±3.0 d(P<0.001)、8.4 d±6.2 d和12.1 d±6.8 d(P<0.05)、11.6 d±4.2 d和16.2 d±6.0 d(P<0.01)。结论在保护性措施下,高渗盐水合用大剂量利尿剂治疗顽固性心力衰竭并低钠血症,其低钠血症和心功能改善快,水肿消退快。Objective To investingate the method,efficacy and safety of hypertonic with high doses uretic on refractory heart failure eomplicated with hyponatremia.Methods 12 refractory heart failure patients were divided randomly into observational group and control group. On a basic routine therapy,the control group applied with high doses uretic meter hypertonic and sodium nitroprusside.Results The serum sodium increased more significantly in control group after one week, the improved time of the serum sodium, the heart function, the dismission of edema were shorted obviously. Conclusion Associated application with hypertonic with high doses uretic on refractory heart failure complicated with hyponatremia, the hyponatremia and heart function were improved quickly,the edema dismissed faster.
