蛛网膜下腔置管脑脊液置换方法的临床比较

摘　　要：目的比较采用单腔深静脉导管和麻醉用硬膜外导管实施蛛网膜下腔置管脑脊液(CSF)置换对蛛网膜下腔出血(SAH)的治疗效果。方法39例患者随机分成两组,分别通过向蛛网膜下腔置入深静脉导管(V组)和硬膜外导管(E组)置换CSF,观察两组在术后24h、72h和大于72h(4周内)的疗效,并观察两组CSF置换次数及置管并发症的发生情况。结果V组在术后24h和72h的缓解例数大于E组(P<0.05),术后4周内发生迟发性脑血管痉挛(DCVS)次数小于E组,CSF置换次数、导管阻塞及脱出次数均小于E组。结论采用单腔深静脉导管实施CSF置换安全、简便,治疗效果好。[Objective] To compare the clinical efficacy of cerebrospinal fluid （CSF） exchange for subarachnoid hemorrhage （SAIl） with playing deep vein catheter or epidural tube into subarachnoid. [Methods] 39 patients undergoing cerebrospinal fluid exchange were divided randomly into two groups. In group V we played a deep vein catheter into subarachnoid and in group E we played an epidural tube into subarachnoid. The therapeutic effect were recorded at 24 h, 72 h and 〉72 h （within four weeks） of postoperation. The times of CSF exchange and the complication were observed. [Results] The relieved patients were more in group V than in group E （P 〈0.05） at 24h, 72h of postoperation. The patients with cerebral vasospasm （DCVS） within four weeks of postoperation were less in group V than in group E. The times of CSF exchange and closure or shedding of the catheter were all less in group V. [ Conclusion] CSF exchange for subarachnoid hemorrhage with playing a deep vein catheter into subarachnoid is an effective and safe treatment.
