可调钠与超滤曲线模式透析在水负荷过多患者血透中临床观察

摘　　要：目的探讨联合应用可调钠与超滤曲线对长期血液透析水负荷过多患者的价值。方法选择透析时间>5个月、透析间期体重增加>3.5kg患者20例,每例患者先进行标准血液透析(SD),后采用钠及超滤曲线呈线型下降模式透析(SP+UP)各10次,观察记录每例患者每次血液透析中低血压、头晕恶心呕吐、肌肉痉挛发生频率,并监测最后3次血透前后血钠浓度的改变。结果SP+UP组低血压、肌肉痉挛发生频率均明显小于SD组,而血钠浓度的改变与SD组差异无显著性。结论可调钠和超滤曲线联合使用能有效减少水负荷过多血液透析患者低血压、肌肉痉挛的发生。[Objective] To investigate the value of combining sodium profile and ultrafitration profde in the patients with overhydration in the interhemodialysis. [Methods] To select 20 patients who received hemodialysis〉5 months and the weight increase〉3.5 kg in the interhemodialysis. Each patient was received standard hemodialysis （SD） for ten times firstly and then received sodium and UF linear descend profde hemodialysis （SP＋UP） for ten times. To observe and record the incidence rate of hypotension muscle cramp during each dialysis. To monitor all patients＇ concentration of serum sodium before and after hemodialysis for the latest three times. [Result] The incidence rate of hypotension muscle cramp reduced significantly and the UF volume increased significantly after receiving SP＋UP, but there was no obviously difference in the electrolyte and acid-base toxin after receiving SP＋UP and receiving SD. [Conclusion] United usement of sodium profde and ultralitration profde can effectively reduce the incidence rate of hypotension and muscle cramp in the patients with overhydration in the interhemodialysis.
