摘 要:目的了解本地区MRSA菌株SCCmec基因型分布和各种基因型的耐药特征。方法用PCR扩增SCCmec分型的特征性基因,对MRSA菌株分型;采用Vitek32微生物分析仪检测MRSA对抗菌药物敏感性。结果84株MRSA菌株中包括SCC-mecⅡ型9株(10.7%),SCCmecⅢ型70株(83.3%),SCCmecⅣ型5株(6.0%)。所有菌株对利福平的耐药率均较低,对万古霉素和利奈唑烷完全敏感,各型别间无显著差异(P>0.05),对其他非β-内酰胺类抗菌药物的耐药率由高到低分别为SCCmecⅢ型、SCCmecⅡ型和SCCmecⅣ型,SCCmecⅡ型和Ⅲ型MRSA菌株表现为多重耐药,其耐药率显著高于SCCmecⅣ型(P<0.05)。结论SCCmecⅢ型菌株为本地区MRSA主要流行菌株,该型菌株对抗生素呈多重耐药。Objective To investigate the distribution of SCCmec genotypes and the antimicrobial resistance of methicillin-resistant staphylococcus aureus (MRSA) with various genotypes in the local area. Methods SCCmec types were determined by PCR. The susceptibility to antimicrobial agents was tested by Vitek32 system. Results Among the 84 isolates of MRSA,9 ( 10.7% ) were belonged to SCCmec Ⅱ,70 (83.3%) were SCCmec Ⅲ ,and 5 (6.0%) were SCCmec Ⅳ. Analysis for drug resistance showed all the strains were low rifampin-resistant and completely sensitive to vancomycin and linezolid. There was no significant difference for the resistance rate among various SCCmec genotypes ( P 〈 0.05 ). The resistance rates to other non-β-lactamase antibiotics in the tested stra.ins were as follows in order: SCCmec Ⅲ 〉 SCCmec Ⅱ 〉 SCCmec Ⅳ. The strains with SCCmec Ⅱ and SCCmec Ⅲ were multi-resistant and their resistance rates were higher than SCCmec Ⅳ ( P 〈 0.05 ). Conclusion MRSA with SCCmec Ⅲ genotype which showed multiple resistance to antibiotics may be the major epidemic isolates in the local area.
