摘 要:目的:研究颅脑损伤后不同阶段脑脊液(CSF)和血清中C-反应蛋白(CRP)的动态变化与脑损伤病情发展的关系。方法:将46例患者分成重型和轻中型分别为22例和24例并与10例正常(志愿者)对比,采用免疫浊度法在第1、3、7、14天时测定脑脊液和血清中CRP的含量变化,用统计学方法分析其数据。结果:重型组CSF和血清中CRP含量在受伤后1天内明显升高,7天达到高峰,维持14天,其水平均高于相应时段的轻中型组(P<0·05),两组均明显高于对照组(P<0·01),伤后到高峰期CSF中的含量显著高于血清(P<0·05);轻中型组中CRP水平在受伤后1天内明显升高,3天达到高峰,以后逐渐下降,14天时接近正常水平。结论:急性颅脑损伤后脑脊液和血清中早期CRP的水平与颅脑损伤的严重程度密切相关。CSF和血清中CRP是重型颅脑损伤患者急性期脑细胞损伤敏感指标,可以作为重型颅脑损伤患者危险性的独立预测因子,对判断病情、治疗效果及估计预后有重要意义。Objective : To explore the development and changes of the C-reactive protein(CRP)in c erebrospinal fluid(CSF)and serum after brain injured,and investigate the relationship between of CRP and the brain injury. Methods:Expressions 46 cases and 10 normal with CRP and CRS by immunoturbidimetric assy. Results:The content of CRP was increased sharply with sever group at 1st day, attaining peak in 7 days up 14 days. A significant difference was noted between CSF and CRP levels in patients with sever injury and moderate injury(P〈0.05) ,two series of all obvious over comparisons series(P〈0. 01 ) ,the injury afterwards reaches the peak stage content among the CSF remarkably is more than serum(P〈0.05) ; The CRP level in light middle-sized series is obviously ascended inner place rear 1 day after wounded,attaiing the peak in 3 days,and drops one by one afterwards, 14 days were normal levels. Conclusions: These results support the use of serum CRP in the CSF for early estimation of the degree of brain injury and prognosis of patients with acute brain injury.
