摘　　要：目的了解2型糖尿病患者血浆脑钠素(BNP)水平的变化及其与血压升高程度的关系。方法将73例2型糖尿病患者分为糖尿病正常血压组(DM组,37例)和糖尿病高血压组(DM+HT组,36例),将35例高血压未合并糖尿病患者作为HT组,25名健康体检者作为C组．测定BNP等指标。结果DM+HT组的BNP水平为(18．1±7．5)pg／mL,显著高于HT组的(13．9±6．8)pg／mL和DM组的(12．5±6．1)pg／mL(P值均< 0．01),DM+HT组和HT组均显著高于C组的(10．1±3．6)pg／mL(P值均<0．01)。BNP与收缩压、舒张压均呈正相关(r=0．4513和0．314 2,P值均<0．05)。DM+HT组中Ⅰ、Ⅱ、Ⅲ级高血压者的BNP水平分别为(13．1±5．1)、(14．3±4．9)和(23．7±10.3)pg／mL,均显著高于DM组(P值均<0．01)。结论高血糖可加重高血压患者心室功能异常。检测BNP水平有利于早期发现2型糖尿病患者的心脏负荷增加。Objective To investigate the changes of plasma brain natriuretic polypeptide（BNP） level in type 2 diabetic mellitus（T2DM） patients and its relationship with blood pressure. Methods All subjects were grouped as controls（C）, 25 cases, hypertensive without diabetes（HT）, 35 cases, DM with normotension（DM）, 37 cases and DM with hypertension（DM＋ HT）, 36 cases. The concentrations of plasma BNP and clinical parameters were evaluated individually. Results （1）The levels of fasting blood glucose were higher in diabetic groups than those in group C and group HT, but showed no significant difference between diabetic groups. The levels of diastolic blood pressure（DBP） and systolic blood pressure（SBP） were much higher in HT and DM＋ HT groups than those in the other groups. （2）The BNP levels in DM＋ HT and HT groups increased more significantly than those in group C and the DM group. （3）The BNP value was positively correlated with the levels of DBP and SBP in all T2DM patients. Conclusion This study suggests that BNP may be one of the markers of maeroangiopathy in T2DM patients. Chronic hyperglycemia may deteriorate the ventrieular function in hypertensive patients. Measurement of BNP maybe helpful for early discovery of ventricular overload. （Shanghai Med J, 2007, 30：340-342）
