移植肾功能异常者与正常者外周血CD8^+CD28^-T细胞水平的比较

摘　　要：目的探讨肾移植受者外周血CD8+CD28-T细胞含量对移植肾功能的影响。方法将接受同种异体肾移植后6~30个月,血清肌酐>133μmol/L的患者25例作为肾功能异常组,同期肾移植手术后血清肌酐≤133μmol/L患者25例为肾功能正常组,两组间根据移植后时间、性别、年龄、手术方式及所用免疫抑制方案等因素进行配对。流式细胞仪测定两组外周淋巴细胞中CD8+CD28-T细胞含量。结果肾功能异常组外周血CD8+CD28-T细胞显著低于肾功能正常组。结论肾移植后外周CD8+CD28-T细胞对移植肾功能具有保护作用。Objective To study the effect of peripheral CD8^＋ CD28^- T lymphocytes on the graft function in renal transplant recipients. Methods Twenty - five allograft renal transplant recipients, median age 47.2 years （29 - 60）, who were 6 months to 30 months after operations and with abnormal serum creatinine （ 〉 133 μmol/L）, were tested for the percentage of CD8^＋ CD28^- T lymphocytes in peripheral blood lymphocytes by flow cytometry. Another 25 renal transplant recipients with normal serum creatinine （ ≤133μmol/L） were also tested for the CD^＋8 CD28^- T lymphocytes as paired control. Each pa/r between the two groups was matched in age, gender, time after transplantation and the regimen of immunosuppression. All cases received triple immunosuppression either with cyclosporine A, mizoribine and prednisone or with tacrolimus, mycophenolate mofetil and prednisone. Results The percentage of CD8^＋ CD28^- T lymphocytes in the group with abnormal serum creatinine was significantly lower than that in the group with normal serum creatinine. Conclu- sion CD8^＋ CD28^- T lymphocytes may have protective effect on graft function in renal transplant recipients.
