摘　　要：目的:评价采用颏下动脉皮瓣联合颈部扩张皮瓣治疗颏部瘢痕的临床效果及其应用价值。方法:2003年8月～2006年7月,对14例颏部瘢痕患者采用分期手术修复颏部瘢痕,Ⅰ期手术在颈部颏下动脉供瓣区下方设计埋置皮肤软组织扩张器,术后定期注水;Ⅱ期手术取出扩张器,切除颏部瘢痕组织,根据创面大小设计颏下动脉皮瓣,转移覆盖创面,供瓣区采用颈部扩张皮瓣修复。颏部瘢痕切除大小为4.5cm×3.0cm～7.5cm×4.5cm。修复所用颏下动脉皮瓣大小为5.5cm×3.8cm～8.0cm×6.0cm。结果:术后所有患者颏下动脉皮瓣和颈部扩张皮瓣血运良好,无皮瓣坏死或皮瓣危象,创面7～10天拆线,后期加强功能锻炼。随访3～30月,平均11月,外观无口角歪斜,颈部功能活动自如,患者及家属满意。结论:采用颏部临近的颏下皮瓣修复颏部瘢痕,供瓣区利用颈部扩张皮瓣予以修复,外观功能均可达到满意的效果,该手术方法简便,不遗留新的瘢痕,是一种治疗颏部瘢痕的理想方法。Objective To evaluate the clinical effect of submental artery skin flap and expanded cervical flap were uesed on chin scar. Methods From Aug 2003 to July 2006, 14 patients with chin scar were treated at stages. At fist stage, different size of soft tissue expander was implanted in cervix next to the chin scar according to preoperation design. At second stage, the soft tissue expanders were taken out, the chin scar was removed, the submental artery skin flap was designed and transferred to the wound according to the size of the wound, and the donor site was covered with expanded cervical flap. The sizes of the scar were from 4.5cm×3.0cm to 7.5 cm×4.5cm, and the submental artery skin flap area ranged from 5.5 cm×3.8cm to 8.0cm×6.0 cm. Results The submental artery skin flap and expanded cervical flap survived well in all patients, without flap necrosis and vessels risk after operation. The suture was removed after 7-10 days. After 3-30 months follow-up, no mouth deflection was observed, the function of neck was well, and the patients were satisfactory with the operation results. Conclusion It can achieve satisfied result to use local submental artery skin flap repairing chin scar and expanded cervical flap repairing donor flap. This method is a simple and ideal method to repair chin scar without leaving new scar.
