摘 要:目的对不同骨密度下股骨假体类型选择进行模拟对比力学实验,从而确定不同年龄段患者临床全髋关节置换术选择股骨假体的标准,为患者选择股骨假体类型提供骨量上的理论依据。方法利用Solidworks计算机软件建立人体股骨三维有限元模型。将不同年龄段骨质的密度及弹性模量分别输入计算机,模拟人体单足站立状态。测量2种人工股骨假体在不同股骨密度下的应力分布、界面应力,以及初始微动3个生物力学特性。结果生物型股骨假体上的整体应力分布,男性30~40岁组和女性30~40岁组总体应力分布相对较均匀,初始微动变化较小。骨水泥型股骨假体上的整体应力分布,在男性60~70岁组和在女性50岁组相对较均匀,初始微动变化较小。结论男性55岁以内和女性50岁以内采用生物型优于骨水泥型人工股骨假体的定量性标准,为临床工作提供了理论依据。Objective To choose the proper type of femoral prosthesis for the patients according to the different hone density through finite- element analysis on the reconstructed femoral hone with different hone density. Methods The three - dimensional finite element model of the femur was established by Solidworks software. Then the human one - foot standing condition was simulated and the pressure was added on the model that was under the different hone density to analyze the Mises, shear force, micro - motion distributing along the femur and confirm the proper prosthesis for different patient with different hone density. Results For the uncemented prosthesis, the best load distribution appeared in the male aged between 30 and 40 years old and the female aged between 30 and 40 years old with the minimal micro- motion and press load. For the cemented femoral prosthesis, the best load distribution appeared in the male aged between 60 and 70 years old and the female of 50 years old group with the minimal micro- motion and press load. Conclusion The uncemented prosthesis stem is better than the cemented one for male of younger than 55 years old and female of younger than 50 years old, which is a quantitative criterion for the clinical practice.
