摘 要:目的 探讨以携带有人骨形态发生蛋白(BMP)-2基因片段的复制缺陷重组腺病毒(Ad—BMP-2)转基因修饰的骨髓间充质干细胞(BMSCs)作为种子细胞,以脱细胞脱钙骨基质材料为支架构建组织工程人工骨用于兔腰椎横突间融合的可行性。方法选择18只日本大耳白兔,在L5,6行单节段双侧横突间融合术,依植入融合材料分为3组:A组为转基因修饰的组织工程骨组;B组为自体髂骨组;C组为单纯支架组。每组12侧。术后12周取出融合节段脊柱,通过大体观察、组织学、影像学及生物力学等方法检测脊柱融合情况。结果三组横突间融合成功率分别为84%(10/12)、75%(9/12)和8%(1/12);组织学、影像学及生物力学检测证实,A、B组成骨活跃,脊柱融合的硬度和强度显著高于C组。结论 转基因修饰的组织工程骨用于脊柱融合可以达到很好的治疗效果。Objective To study the feasibility of rabbit posterlateral lumbar intertransverse process of spinal fusion with tissue engineered bone graft substitute, in which the bone marrow stem ceils (MSCs) transfected with Ad-BMP-2 were served as the seed cells and the allogenetic acellular and decalcified bone matrix as tissue engineering scaffold. Methods A total of 18 big ear Japanese rabbits underwent spinal fusion of bilateral intertransverse process at L5,6. All cases were divided into three groups according to implantation materials, ie, tissue engineered bone graft substitute seeded with gene transfected MSCs (Group A, n = 12), autogenous iliac crest bone (Group B, n = 12) and allogenetic acellular and decalcified bone matrix served as tissue engineering scaffold ( Group C, n = 12). The result of spinal fusion was assessed by manual palpation, histology, radiography and tensile biomechanical test 12 weeks later. Results Posterlateral lumbar intertransverse process reached bony. fusion in Groups A and B. The fusion rates of Groups A, B and C at 12 weeks after operation were 84% (10/12), 75% (9/12) and 8% (1/12) respectively. Tensile strength and stiffness of the fused spine in Groups A and B were statistically higher than those of Group C, which was proved by histological, radiographic and biomechanical tests. Conclusion The tissue engineered bone grafts substitute treated with gene transfection is beneficial to spinal fusion of posterlateral lumbar intertransverse process in rabbits.
