经脐单孔法腹腔镜治疗小儿腹股沟斜疝的临床研究

摘　　要：目的探讨经脐单孔法腹腔镜治疗小儿腹股沟斜疝的临床应用价值。方法采用经脐单孔法腹腔镜治疗小儿腹股沟斜疝98例,并与小儿腹股沟疝的传统手术在手术时间、肠功能恢复时间、住院天数、术后并发症等方面进行比较。结果经脐单孔法腹腔镜治疗小儿腹股沟斜疝手术过程顺利,适合于3岁以上的患儿,其平均手术时间为(12±5)分钟,肠鸣音恢复时间为(4.8±0.3)小时,住院时间2天,较传统手术具有明显优势。术后随访3个月-4年末见肠管损伤、输精管损伤、膀胱损伤、睾丸萎缩等并发症,复发1例。结论经脐单孔法腹腔镜治疗小儿腹股沟斜疝是一种安全的微创手术,具有损伤小、恢复快、住院时间短、术后无疤痕等优点,值得推广应用。Ojective To study the clinical value of single port laparoscopic surgery of inguinal hernia in children. Methods 98 child inguinal hernia patients were treated by single port laparoscopic surgery from Apr. 2002 to Nov. 2006, and compared with normal inguinal hernia surgery in operational time, recovery time of intestinal function, in - patient time and surgical complications. Results Surgical procedure was done for all under three year- old children. The average operational time was 12 ＋ 5min, the recovery time of intestinal function was 4.8 4- 0.3h and in - patient time was two days. After three month to four years following- up, there were not side effect such as colon injury, vas deferens injury, bladder injury and testicle atrophy. One patient was failure. Conclusion Single port laparoscopic surgery of inguinal hernia is a safe, minimally invasive operation with little injury, quick recovery, short in- patient time in children. It is worth to be used widely.
