一种总体止血能力的检测方法及其应用

摘　　要：目的建立一种能够反映总体止血能力的检测方法,用于评价高凝状态和凝血障碍。方法在纤溶酶原激活物UK存在下,以低浓度组织因子启动血浆凝固,在340nm测量纤维蛋白形成与降解过程中的吸光度变化,获得的凝血-纤溶曲线经计算机分析处理,求出总体止血能力(overall haemostatic potential,OHP)、总体凝血能力(overall coagulation potential,OCP)和总体纤溶能力(overall fibrinolysis potential,OFP)等参数。结果发现恶性肿瘤、妊娠和冠心病普遍存在高凝状态,表现在OCP和OHP值增高和/或OFP值降低;血友病OCP和OHP明显减低。结论这种总体止血能力检测方法能够反映机体止血平衡,不仅可作为高凝状态的检测手段,还可用作凝血障碍的筛选手段。Objective To establish an overall haemostatic potential assay and evaluate its ability to indicate the hypercoagulable state and the coagulopathy. Methods In the present of plasminogen activator （UK） the plasma clotting was triggered by the low concentration of tissue factor. The time course of fibrin formation and the fibrin degradation was measured by a spectrophotometer at 340 nm. The opacity curve obtained for coagulation-fibrinolysis was processed by a personal computer. The related parameters including the overall haemostatic potential （OHP）, overall coagulation potential （OCP） and the overall fibrinolysis potential（OFP） were established. Results It was found that OHP and OCP were increased and/or OFP was decreased in the patients of cancer, normal pregnancy and coronary heart disease. The significant decreased OCP and OHP was found in the hemophilia patients. Conclusion It is therefore concluded that the overall haemostatic potential assay may indicate the body haemostatic balance. It is not only used for evaluation of the hypercoagulable state , also for the screening of coagulopathy
