摘 要:目的阐明陕南燃煤型砷皮肤损伤的危险因素。方法依据地方性砷中毒的诊断标准对陕西省安康市汉滨区2个村的198例居民进行现况调查。在单因素分析剔除对燃煤型砷皮肤损伤相关性小的因素的基础上,采用主成分分析方法消除因素间的共线性,再用logistic逐步回归模型分析筛选出燃煤型砷皮肤色素异常改变的主要危险因素。结果单因素分析显示,21个变量中年龄、性别、居住年限、接触高砷煤时限、发砷、尿砷、血砷、尿硒、血硒、发硒和吸烟等11个变量与燃煤型砷皮肤损伤有关,经主成分分析消除因素间的共线性后再引入logistic回归模型,筛选出接触高砷煤时限、年龄、血砷负荷、居住年限、尿砷负荷和发砷负荷等6个主要危险因素。结论接触高砷煤时限、年龄、血砷负荷、居住年限、尿砷负荷和发砷负荷是陕南燃煤型砷皮肤损伤的主要危险因素。To explore risk factors of the patients with skin lesions from burning high-arsenic-contaminated coal in southern Shanxi Province. Methods An epidemiological cross-sectional study was conducted in two villages of Hanbin District, Ankang City, Shanxi Province, where 198 participants were randomly selected. The parts of factors which were not significantly different were rejected via the single variable analysis and the collinear relationships were removed between the other factors using principal components analysis, and then the major risk factors of the skin lesions from buming high-arsenic-contaminated coal were obtained from stepwise logistic regression. Results The single variable analysis showed that 11 influential variables were significantly associaed with coal-buming-arsenic-associated skin lesions, while principal component and logistic regression analysis showed that 6 variables could be the principal, which were period of burining arsenic-contaminating coal, ege, the arsenic content in blood, resident duration, the arsenic content in urine and hair, respectively. Conclusion The results suggested that the major risk factors of the skin lesions related to burning coal-contaminating arsenic should be period of burning arsenic-contaminating coal, age, the arsenic content in blood, resident duration, the arsenic content in urine and hair.
