摘　　要：目的:探讨医用生物蛋白胶在腹腔镜肝切除术肝断面中的应用。方法:将40例肝切除术患者随机分为腹腔镜组与对照组。腹腔镜组行腹腔镜肝切除,经超声刀断肝,腹腔镜下喷涂医用生物蛋白胶方法处理肝断面;对照组行常规腹腔镜肝切除术。术后通过腹腔引流管观察出血量及胆瘘情况。结果:腹腔镜组手术全部成功,术后恢复顺利,术后引流量、引流液红细胞计数及血红蛋白含量、住院时间明显低于对照组(P<0·01),胆瘘1例。结论:腹腔镜肝切除术中肝断面应用生物蛋白胶喷涂,简化了手术方法,有效防止渗出和胆瘘的发生,可作为肝切除术的一种新选择。OBJECTIVE： To discuss the application of biomedical fibrin glue in laparoscopic hepatectomy（LH） .METHODS： 40 patients undergoing LH were randomly divided into biomedical fibrin glue group（trial group） and control group： the patients in trial group underwent LH, transection of liver by ultrasound knife, with the transected liver surface sprayed with biomedical fibrin glue guided by laparoscope; the control group underwent routine LH only.The postoperative blood loss and bile leakage were observed by T- tube drainage.RESULTS： In the biomedical fibrin glue group, the operations for all the patients were successful, the postoperative recovery was smooth,the postoperative drainage volume,RBC in the drained fluid,level of hemoglobin and length of hospital stay were all significantly lower than in the control group（P 〈 0.01）, and only 1 case had bile leakage.CONCLUSION：The application of biomedical fibrin glue in the transected liver surface of patients undergoing LH has helped simplify the operation and effectively lowered the incidence of exudation and bile leakage, which thus can be served as a new choice for hepatectormy.
