儿童副鼻窦支气管炎治疗方案探讨

摘　　要：目的探讨儿童副鼻窦支气管炎的治疗方案、分析对儿童慢性咳嗽的认识及诊疗对策。方法对72例儿童副鼻窦支气管炎患儿作全程疗效随访观察,72例中男59例、女13例,随机分成三组:TFH组(29例)用头孢特仑脂(富山龙)+N-乙酰半胱氨酸(富露施)+核酪口服液治疗,ZFH组(17例)用阿奇霉素(希舒美)+富露施+核酪口服液治疗,对照组(26例)按常规用药。应用统计学方法分析疗效结果。结果TFH组和ZFH组疗效均优于对照组,且ZFH组优于TFH组。ZFH组和TFH组总有效率分别达100%和79.3%,与对照组比较差异均有统计学意义。以10d为一个疗程,儿童副鼻窦支气管炎的疗程大约需要6～8个疗程(2个月以上)。结论对儿童副鼻窦支气管炎应采用综合性治疗,在抗生素应用的基础上应配合应用祛痰剂和免疫增强剂。ZFH方案是治疗儿童副鼻窦支气管炎的最佳方案。Objectives To explore the therapeutic protocol for childhood sinobronchitis and discuss the strategy of diagnosis and treatment of childhood chronic cough. Methods Therapeutic effect of 72 children （including 59 males and 13 females） with sinobronchitis were monitored in the whole course. They were randomly divided into three groups actorcling to the therapeutic protocols, TFH group （29 cases. The drugs used in this group were Tomiron, Fluimucil, and Haachos）, ZFH group, （17 cases. The drugs used in this group were Zithromax, Fluimucil, and Haachos） and control group （26 cases. Only common drugs used） . Therapeutic efficacy was studied with statistical analysis. Results The result of TFH group and ZFH group were much better than the control group. And the result of ZFH group was better than TFH group. The total effective rate of ZFH group and TFH group was 100% and 79.3% respectively, with significant difference from control group. With ten days as a treatment course, about 6 to 8 courses were needed for completely recovery （more than 2 months） . Conclusions Combined therapeutic protocol should be applied for childhood sinobronchitis. The expectorant and the immunoenhancer should be added on the antibiotics. ZFH protocol is the best therapeutic one for treating childhood sinobronchitis.
