四安胶囊治疗胰岛素抵抗综合征的临床研究

摘　　要：中药复方制剂四安胶囊，对中医辨证属肝火亢盛、痰瘀互结的胰岛素抵抗综合征患者，能显著改善证候，总有效率９３．９％，证候积分由治疗前的２４．３±４．１降至１１．３±３．０（Ｐ＜０．０１）；降低收缩压及舒张压（Ｐ＜０．０１），但与对照组相比无显著性差异（Ｐ＞０．０５）；降低血糖（Ｐ＜０．０１），与对照组相比有显著性差异（Ｐ＜０．０５）；调节血脂比例，使ＴＧ、ＴＣＨ、ＬＤＬ、血脂指数及ＡＰＯＢ显著降低（Ｐ＜０．０５～０．０１）；降低血胰岛素水平（Ｐ＜０．０１）；显著改善患者的血液流变性，使全血比粘度、血浆比粘度和红细胞压积降低，缩短红细胞电泳时间，增加电泳率（Ｐ＜０．０５～０．０１），其中对血浆比粘度的降低优于巯甲丙脯酸（Ｐ＜０．０５）；改善微循环，使动脉口径增加，静脉口径减小（Ｐ＜０．０５），对微循环血色、流态、流速有显著改善作用（Ｐ＜０．０５），而巯甲丙脯酸仅能减小静脉口径及改善血流状态（Ｐ＜０．０５）。In order to find the effective drug to treat Insulin Resistance syndrome,we manufanctured Si'an Capsule(SA)and made clinical study which were performed on 64 patients with the diagnosis of Insulin resistance syndrome.Every patient was classified to hyperactivity of the fire of the liver or phlegn accompanied with stagnation of blood according to TCM syndrome differentiation.They were divided randomly into two groups:the treatment group(33 patients)and the control group(31 patients) with the treatment of SA or Captopril respectively.The results were as follows:SA could decrease blood pressure(BP) from 22.84±2.19/12.9±1.37kPa to 19.68±1.93/10.9±1.35kPa on patients.The effect were significant compared with that of before treatment (P<0.01) and had no difference with the control group(P>0.05).SA could lower the serium content of Insulin on treatment group(P<0.01).The improvement was superior than that in the control group (P<0.05).SA could drop blood sugar on patients (6.2±1.8 to 5.1±1.4).The difference with the control group was significant(P<0.05～0.01).SA could adjust the serium contents of lipoprotein and apoliprotein on patients.The TCH,TG and LDL decreased remarkedly,APOB and index of lipoprotein dropped evidently on patients.SA could drop whole blood relative viscosity(high share rate and low share rate),plasma relative viscosity,hematocrit and shorten RBC electrophoresis period.The improvement of plasma relative viscosity was superior than that of the control group(P<0.05).Meanwhile,SA could improve microcirculation such as increasing attery width and reducing vein width(P<0.05).The colour,the state and velocity of blood were improved too (P<0.05).Only the width of vein and state of blood were improved in the control group(P<0.05).
