皮肤软组织扩张联合深层纤维包膜在肌腱粘连修复中的应用

摘　　要：目的 将皮肤软组织扩张联合深层包膜应用于肌腱损伤后修复的效果。方法 利用软组织扩张术，首选重建皮肤软组织，扩张器取出时，切除肌腱附近的瘢痕组织，同期应用深层包膜进行肌腱粘连松解术，以预防术后肌腱的粘连。结果 术后6个月随访，重建的皮肤在色泽、质地、弹性、耐磨程度、活动灵活度等方面均与正常接近，同期进行的肌腱松解术优良率92.3％（TAM标准）。结论 皮肤软组织扩张术不仅为肌腱损伤后修复提供高质量的皮肤，同时能恢复局部双层膜性结构。Objective To use the skin and soft-tissue expansion with deep-fibrous capsul in tendon repair. Methods The skin and soft-tissue expansion was performed in 12 patients. Firstly, the skin and soft-tissue was expanded by tissue expander, after taking out the expander, cut away the scar tissue around the tendon and tenolysis was. preformed simultaneously to prevent the adhesion of the tendon. Results Follow up was performed at 6 months post-operatively, the reconstructed skin was near to normal one in coulur, texture,elasticity and wear resisting. The overall excellent and good rate was 84.6% （ATM standard） in tenolysis. Conclusion The skin and soft-tissue expansion provides high quality of skin and deep-fibrous capsul coverage for tendon repair to make it attain the double-decked structure of the film. It can avoid the tendon adhesion once again and gain good clinical results in tendon repair.
