摘  要:目的:研究颅脑爆震伤早期病理生理机制。方法:30只犬随机分为3个不同爆震距离组(4mm、8mm和12mm组),应用爆炸球冲击波致伤。观察比较犬伤后生理指标的变化及脑组织的病理改变。结果:颅脑爆震伤后犬出现呼吸骤停,爆炸距离越近,呼吸暂停时间越长,三组比较有显著性差异(P<0.05)。伤后犬出现心率下降及血压升高,但组间比较无显著性差异(P>0.05)。三组犬均有程度不同的脑挫裂伤、硬膜下血肿和脑干出血等病理改变。结论:颅脑爆震伤后出现呼吸、心率、血压等生命体征改变,间接反映了神经中枢所受到的损害,给予呼吸、循环辅助治疗十分必要;颅脑爆震伤后普遍发生硬膜下血肿,早期积极干预可能改善预后。Objective:To investigate the early physiologic and pathologic changes of brain explosive injury. Methods:Thirty dogs were incurred injuries from spherical explosive and randomly divided into three groups according to different distance explosive injury(4,8 and 12 mm). Physiological and pathological changes were observed. Results:After blast, there was breathing holding, the nearer the instance ,the longer the time of breathing (BR) , the time of BR was different significantly in the three groups(P 〈0.05). The decrease of heart rate (HR) and rise of blood pressure (BP) were observed, but the extent of which were not different significantly in the three groups (P 〉 0.05 ). There were different extent of contusion and laceration of the brain, subdural hematoma, brain stem hemorrhage and etc. Conclusion :After blast,the changes of vital sign (BR,HR and BP) indirectly reflected the damage to the nerve center. It' s necessary that adjunctive therapy for respiration and circulation be instituted. The subdural hematoma are general ,and early intervention may improve the prognosis.
