流式尿液分析在筛检妊娠早期无症状尿路感染中的应用

摘　　要：目的应用UF-100尿液分析仪（以下简称UF-100）检测妊娠早期妇女中段尿，以中段尿定量细菌培养作为金标准，探讨UF-100在筛检妊娠早期妇女无症状尿路感染（AUTI）中的可靠性。方法对667份妊娠早期妇女中段尿标本，进行细菌培养菌落计数与UF-100流式尿液分析进行比较。结果51例培养出细菌，占7．7％；在UF-100细菌计量为≥2750／μL，白细胞≥20/μL为阳性时，待检标本的特异性（Sp）和阴性预示值（NPV）最大，分别为99．4％、99．5％，但此时敏感性（Se）和阳性预示值（PPV）稍低，分别为94．0％、92．2％。结论快速筛检妊娠早期无症状尿路感染者用UF-100是可行的；诊断妊娠早期妇女无症状尿路感染仍须做细菌培养。Objective To compare the reliability of flow cytometer UF-100 in screening asymptomatic urinary tract infection （AUTI） in early pregnant women with quantitative urine culture of mid-stream urine as a golden criterion. Methods 667 fresh midstream clean-catch urine samples were collected in accordance with standard guidelines. Each midstream urine specimen was examined by cultures and counts of bacterial colonies, flow cytometer UF-100 and then to compare the results of them. Results Fifty-one patient （7.7% 51/667） had cultured uropathogen. The specificity （Sp） and negative predictive value （NPV） was obtained with bacteria at ≥2750/μL, white blood cell at ≥20/μL, which was 99.4% and 99.5% respectively, but meanwhile the sensitivity（Sen） and positive predictive value （PPV） was a little decrease, which was 94.0% and 92.2% respectively. Conclusions Flow cytometer UF-100 are sufficiently useful to screen for AUTI rapidly, but the bacteriuria culture must be done in diagnosing AUTI.
