云南省家鼠鼠疫流行特征与强度分析

摘　　要：目的分析云南省1982-2004年家鼠鼠疫的流行特征与强度。方法收集云南省20多年来鼠疫监测及相关病例资料,运用SPSS 14.0和Map Info及相关软件进行统计分析。结果云南省1982-2004年共有46个县(市)发生动物鼠疫流行,按流行强度的大小经聚类分析可将其分为3类,流行强度以第1类地区德宏州的陇川县、瑞丽市、潞西市、盈江县和临沧市的凤翔区为高发区,分别为20、12、11、16和10疫县次。人间鼠疫波及29个县(市),共确诊鼠疫患者503例。主要分布在多发区和偶发区,以多发区为主。主要传染源为黄胸鼠,传播媒介为印鼠客蚤。结论目前云南省仍处在家鼠鼠疫的活跃期;疫点以散在分布为主,并有局部暴发流行;鼠疫呈现自西向东,由边向内次第发展的严重局面,鼠疫仍然是云南省现阶段主要的公共卫生问题。Objective To analyze the epidemiological character and intensity of commensal rodent plague in recent over twenty years in Yunnan. Methods Collecting data of commensal rodent plague and plague patients in recent over twenty years, and using SPSS 14.0, Map Info and relative software to analyze the data. Results From 1982 to 2004, totally, 46 counties/ cities occurred commensal rodent plague in Yunnan. According to the epidemiological intensity, the plague areas were classified three different type areas by Hierarchical Cluster Analysis. Type Ⅰ areas include 5 counties/cities： Dehong prefecture＇s Longchuang county, Ruili city, Luxi city, Yingjiang county and Lincang prefecture＇s Fengxiang district. Epidemiological intensities were 20, 12, 11, 16, 10 respectively. 503 patients were founded in 29 counties, distributed mainly in type Ⅱ areas and type Ⅲ areas, especially in type II areas. Primary host is Rattus flavipectus, and primary vector is Xenopsylla cheopis. Conclusion At present, commensal rodent plague is still active in Yunnan; Distribution of plague focus is sporadic occurrence meanwhile epidemic outbreak in some part; Commensal rodent plague showed a serious situation： re-emerging from the east to the west, from the border to the core of Yunnan. Plague is still a critical public health problem in Yunnan province.
