摘 要:目的:研究注射用精氨酸七叶皂苷健康人体单剂量给药的药动学。方法:采用剂量递增试验设计,30名健康志愿者随机分为3个剂量组,分别静滴注射用精氨酸七叶皂苷5.77 mg,11.54 mg或23.08 mg,用LC-MS/MS同时测定血浆中和尿样中七叶皂苷主要组分的浓度,计算其主要药代动力学参数。结果:健康志愿者单剂量静滴注射用精氨酸七叶皂苷后,七叶皂苷A组分的Cmax分别为(137.7±33.71),(255.9±41.57)和(431.7±104.52)ng.mL-1;AUC0-t分别为(400.9±138.69),(830.8±235.02)和(1 622.9±613.16)ng.h.mL-1;t1/2Ke分别为(2.46±0.53),(3.34±0.42)和(5.66±0.82)h;24 h尿中七叶皂苷A组分累积排出百分率分别为(4.70±1.47),(5.17±1.35)和(5.53±1.85)%。七叶皂苷B组分的Cm ax分别为(74.5±20.87),(141.1±25.64)和(231.8±63.50)ng.mL-1;AUC0-t分别为(196.3±68.22),(381.2±107.60)和(782.7±321.07)ng.h.mL-1;t1/2Ke分别为(2.13±0.68),(2.96±0.79)和(4.88±1.18)h;24 h尿中七叶皂苷B组分累积排出百分率分别为(2.54±0.71),(2.69±0.59)和(3.12±1.17)%。结论:健康人体单剂量静滴注射用精氨酸七叶皂苷5.77~23.08 mg剂量范围内,七叶皂苷A组分和B组分的Cm ax,AUC0-t和t1/2Ke均随着剂量的增加而增加。Objective:To study the pharmacokinetics of arginine aescinate after intravenous infusion in healthy Chinese volunteers. Methods: A dose-escalation study was conducted in which 30 healthy volunteers were randomized to 5.77, 11.54, and 23.08 mg dose groups. The concentrations of arginine aescinate in plasma and urine were determined by LC/MS/MS. The main pharmacokinetic parameters were calculated using WinNonlin version 5.0. Results: The main pharmacokinetic parameters of an ingredient of aescinate after single doses of 5.77, 11.54, and 23.08 mg arginine aescinate were as follows : C max(137.7±33.71), (255.9±41.57) and (431.7±104.52)ng·mL^-1; AUC0-t(400.9±138.69), (830.8 ±235.02) and(1 622.9 ±613.16)ng·h·mL^-1; t1/2Ke(2.46 ±0.53), (3.34±0.42) and (5.66 ± 0. 82) h ; and 24-hour cumulative urinary excretion rates (4.70 ± 1.47 ) , (5.17 ± 1.35 ) and (5.53 ± 1.85) % . The main pharmacokinetic parameters of B ingredient of aescinate were as follows: Cmax(74.5±20.87), (141.1±25.64) and (231.8 ±63.50)ng·mL^-1; AUC0-t(196.3 ±68.22), (381.2±107.60) and(782.7±321.07)ng·h·mL^-1; t1/2Ke (2.13±0.68), (2.96 ±0.79) and (4.88 ± 1.18) h; and 24-hour cumulative urinary excretion rates (2.54 ±0.71) , (2.69±0.59) and (3.12 ± 1.17) %. Conclusion: C AUC0-t, and t1/2Ke tended to increase with increasing doses of 5.77 mg to 23.08 mg arginine aescinate after intravenous infusion in Chinese healthy volunteers.
