摘 要:目的观察伊曲康唑注射液对于快速控制大鼠播散性毛孢子菌病的疗效。方法48只大鼠随机分为3组,每组16只,建立大鼠播散性毛孢子菌病模型后,ICZ组第1,2天给予伊曲康唑注射液20mg/kg,2次/d,第3~5天1次/d,静注;AMB组给予两性霉素B1mg/kg,1次/d,连续5天,静注;Control组给予静注生理盐水,治疗结束后观察各组存活率、累计存活天数、肝脾等脏器真菌负荷量,并测定血浆β-D-葡聚糖水平。结果ICZ组大鼠存活率为14/16(87.5%),AMB治疗组为12/16(75.0%),对照组为8/16(50.0%),前者与对照组差异有显著性(P=0.02),而后者与对照组差异无显著性(P=0.14)。ICZ组与对照组肝、脾组织真菌菌落计数比较差异有显著性(P均<0.05),AMB组与对照组肝、脾组织真菌负荷量比较差异有显著性(P均<0.05)。ICZ组与AMB组累计存活天数,肝、脾真菌负荷量比较差异无显著性(P均>0.05)。伊曲康唑治疗后与治疗前,以及同期未治疗的对照组血浆(1,3)-β-D-葡聚糖比较差异均有显著性(P均<0.05)。结论初步动物实验结果提示伊曲康唑注射液可有效控制播散性毛孢子菌病的发展,提高存活率,有希望成为两性霉素B不便应用时的替代药物之一。Objective To investigate the effects of itraconazole injection on the murine disseminated trichosporosis model. Methods 48 rats were divided into 3 groups and injection,20 mg· kg^- ,twice daily on the 1^st and 2^nd day,once on the 3^th -5^th days, intravenously ; AmB group : amphotericin B 1 mg · kg^- once daily, intravenously ; Control group were injected intravenously with 0.9% saline solution. All the dead rats and the survived rats on the 14th day were autopsied. The livers ,spleens were weigh- ted ,levigated and inoculated on the plate of Saboraund' s agar. Two days later the colonies were counted. ( 1,3 ) -13-β-D -glucan of the blood samples from orbital veins were detected. Results There were significant differences of the mortality,liver and spleen residual burden,plasma (1,3)-β-D-glucan level between itraconazole treatment group and that of control group( P value were 0. 023, 0. 027,0.03 respectively) , but not between the ICZ group and AM B group ( P value were 0.52,0. 84). Conclusion Itraconazole injection is effective to disseminated trichosporosis ,similar or better than that of amphotericin B ,which can provide a new choice to treat the severe systemic fungi infection.
