氟比洛芬酯用于乳腺肿瘤手术后镇痛效果及安全性

摘　　要：目的:观察氟比洛芬酯用于乳腺肿瘤手术后镇痛的效果及安全性。方法:选择择期乳腺肿瘤手术病人60例,随机分为两组,氟比洛芬酯组(Ⅰ组)30例,曲马多组(Ⅱ组)30例,均采用静脉复合全身麻醉。术毕拔管后Ⅰ组氟比洛芬酯50 mg缓慢iv,Ⅱ组曲马多100 mgim。采用Prince-Henry疼痛评分法评定镇痛效果,记录用药后2 h、4 h、6 h、8 h、10 h、15 h疼痛评分及镇痛时间;监测并记录注药前,注药后10 min、20 min、30 min、60 min、120 min的血压、心率、呼吸频率,SpO2和心电图变化以及镇痛期间不良反应发生情况。结果:两组10 h内各时点疼痛评分均值小于2,镇痛满意率Ⅰ组为83.3%,Ⅱ组为90%;镇痛持续时间Ⅰ组略长于Ⅱ组(12.4±3.5 h vs 11.9±3.6 h)(P>0.05)。术后镇痛期间两组患者呼吸、循环功能均稳定。Ⅰ组不良反应发生率低于Ⅱ组。结论:与曲马多相似氟比洛芬酯用于乳腺肿瘤手术后镇痛是有效的、安全的和可行的。Objective： To observe the efficacy and safety of flurbiprofen axetil for postoperative analgesia of mammary tumor patients . Methods ： 60 patients （ ASA Ⅰ - Ⅱ） scheduled to undergo mammary surgery under general anesthesia were randomly divided into two groups： group I （flurbiprofen axetil group, n = 30） and group Ⅱ （ tramadol group, n = 30）. Patients were given flurbiprofen axetil （50 mg, iv group Ⅰ） or tramadol （ 100 mg, im group Ⅱ ） respectively after opeartion. The analgesia score at 2 h, 4 h, 6 h, 8 h, 10 h and 15 h after injection were assessed by Prince-Henry scale and recorded the analgesia time. The SBP, DBP, HR, SPO2, RR and SpO2 of patients were monitored pre-injection and at 10 min, 20 min, 30 min, 60 min and 120 min post-injection. The adverse reactions were also recorded during analgesia. Results： In two groups, the analgesic score were less than 2 at each time-points in 10 h （ p 〉 0. 05 ）. In group Ⅰ and Ⅱ , the superior rates of analgesia were 83.3% and 90% respectively. The respir- atory and circulative functions of patients were all stable during analgesia. In group Ⅰ, the rate of adverse reaction was lower than that in group Ⅱ. Conclusion： The postoperative analgesic effect of flurbiprofen axetil is effective, safe and feasible for patients of mammary surgery.
