摘　　要：目的总结分析189例非体外循环冠状动脉旁路移植术(OPCAB)的经验体会。方法2002年3月-2006年12月共完成OPCAB189例,占同期冠心病手术的78%。平均年龄(63.1±8.7)岁。其中左主干病变17例,单支血管病变11例,双支血管病变47例,三支血管病变131例。2例经左前外侧切口行单支病变旁路移植术,余187例均为正中切口,平均远端吻合口(3.1±0.7)支。结果全组无住院死亡。平均呼吸机辅助时间6.2h,无围术期心肌梗死。二次开胸止血2例,胸骨愈合不良4例。结论非体外循环下冠状动脉旁路移植术是一种安全、可行、有效的手术方式。Objective To summarize and analyze the clinical experience in 189 patients underwent off- pump coronary artery bypass （OPCAB）. Methods 189 patients with coronary artery heart disease underwent OPCAB grafting from March 2002 to December 2006, aged 38 - 77 years（ 63.1 ± 8.7）years. The ratio of OPCAB to conventional coronary artery bypass grafting was 78%. 17cases with left main lesion, 131cases with triple -vessel disease. The number of distal anastomosis was 3.1 ±0. 7. Results All patients were clinical recovery. Respiration assisted with an average time of 6. 2hours. no peri - operative myocardial infarction. The morbidity included 2 cases reopen for bleeding, and 4 cases delayed wound healing. Conclusion OPCAB suryery is a safe and feasible procedure with lower mortality and morbidity.
