摘  要:目的探讨应用宫腔镜电切术治疗宫颈病变的临床效果。方法2004年12月~2007年1月应用宫腔电切镜切除386例各类宫颈病变。结果386例患者住院83例,门诊病人303例,均一次完成手术。手术时间5~20 min,术中出血3~20 ml。术后病理诊断慢性宫颈炎360例(包括宫颈潴留囊肿宫颈和宫颈及宫颈息肉),宫颈上皮内瘤样病变(cervical intraepithelial neoplasia,CIN)Ⅰ~Ⅱ级25例,术后病理为宫颈原位癌1例。宫颈糜烂及宫颈肥大伴潴留囊肿,治愈273例,治愈率97.4%无效0例,总效率100%。宫颈息肉81例,术后半年无复发。24例CIN术后随访6—12个月未显示CIN病变。结论宫腔镜电切术治疗宫颈疾病能有效降低宫颈癌发病率,积极治疗宫颈疾病。同时CINⅠ~Ⅱ级病变患者随诊满意,疗效肯定。故应用官腔镜电切术治疗宫颈病变的临床效果显著,值得推广。Objective To investigate the clinical effect of hysteroscopic electroresection of cervical lesions.Methods Hysteroscopic electrosurgery of transcervical resection of cervical lesions was conducted in 386 cases from December 2004 to January 2007. Results The procedure was accomplished in the out - patient clinic on one session in all the 303 cases. 83 cases were hospitalized patients. Operation time was 5 - 25min and the intraoperative bloodloss3 - 20ml. Post operative pathological examinations showed 360 cases of chironic cervicitis and 25 cases of cervical intraepithelial neoplasia(CIN)(grade Ⅰ - Ⅱ) .one case was carcinoma in situs of cervix.Cure of cervical erosion and Cervical hypertrophy combined with Cervical retention cyst is 97.4% ,273 cases was cured, the rate of validity is 100 %. 81 cases were cervical polyps,in the following 6months, were founded no recurrence. A follow- up survey was carried out in 24 cases of CIN. it demonstrated no findings of CIN.Conclusions Hysteroscopic electro surgery for cervical lesions can bring down the incidence of the carcinoma of carvical uteri efficiently, it can treat cervical desease actively. The follow - up survey was satisfactory. It has good effect. CIN, Hysteroscopic electrosurgery for lesions gives favorable outcomes, being a procedure worthy of recommendation.
