摘  要:目的探讨早期2型糖尿病肾病尿微量蛋白与幽门螺杆菌(HP)感染的关系。方法75例2型糖尿病患者(T2DM,HP感染组39例,无HP感染组36例)和正常对照组30例尿白蛋白(Alb)、α1-微球蛋白(α1-MG)、血浆同型半胱氨酸(HCY)、血清血管性血友病因子(vWF)和血小板膜表面GMP-140(GMP140)进行了测定。结果①T2DM患者尿Alb、α1-MG和血中HCY、vWF、GMP-140皆显著高于正常对照组(P〈0.05),且HP感染组显著高于无HP感染组(P〈0.01);②HP感染组微量蛋白阳性率显著高于无HP感染组(P〈0.01);③HCY与尿中Alb、α1-MG、血中vWF、GMP-140成正相关系(P〈0.05);经多元回归HP感染是T2DM尿微量蛋白升高的因素。结论HP感染在T2DM发展成早期肾病的过程中可能扮演了重要的角色,起到了引发、加速早期肾病的作用。Objective To Probe into correlation between helicobacter pylori (HP) infection and microalbuminuria in early type 2 diabetic (T2DM) nephropathy. Methods Urinary albumin ( Alb), α1 - microglobulin (α1 -MG), and plasma homocysteine (HCY), and serum von Willebrand factor (vWF), platelet granule membrane protein - 140 (GMP - 140) of 75 patients with T2DM (39 cases with HP infection ,36 cases without HP infection) and of 30 healthy people as control group were determined. Results ①Contents of Alb, α1 -MG,HCY,vWF and GMP- 140 two groups with T2DM were significantly more than those in control group ( P 〈 0.05 ), and those in HP infection group were more markedly higher than those in non - HP - infection group ( P 〈 0.01 ) ; ②The positive rate of microalbuminuria in HP infection group was significantly higher than that in non - HP - infection group ( P 〈 0.01 ) ; ③HCY in T2DM was positively correlated with Alb, α1 -MG,vWF and GMP- 140 ( P 〈 0.05 ). Further multivariate regression showed HP infection was directly related with the level of microalbuminuria in T2DM. Conclusion HP infection play an important role in the development of early nephropathy in T2DM,it induces the acceleration of early nephropathy.
