妊娠期75g口服葡萄糖耐量试验各时段血糖值比值的意义

摘　　要：目的比较葡萄糖耐量异常和正常孕妇75g口服葡萄糖耐量试验（OGTT）各时段血糖值比值,并探讨其临床意义。方法收集1909例孕24-36周孕妇的75gOGTT资料,按美国糖尿病协会诊断标准,将孕妇分为妊娠期糖尿病组（GDM组,n=60）、妊娠期葡萄糖耐量损害组（GIGT组,n=156）和正常葡萄糖耐量组（NGT组,n=1693）。根据75gOGTT中空腹血糖（FPG）及葡萄糖负荷后1、2、3h血糖值（PG1h、PG2h、PG3h）分别计算各组PG3h/FPG、PG2h/FPG、PG1h/FPG、PG3h/PG1h、PG2h/PG1h、PG3h/PG2h比值,并进行统计学分析。结果GDM组与GIGT组各比值之间以及三组间PG3h/FPG比较均无统计学差异（P〉0.05）;GDM组和GIGT组PG2h/FPG、PG1h/FPG均明显高于NGT组（P〈0.05）,而两组PG3h/PG1h、PG2h/PG1h和PG3h/PG2h均小于NGT组（P〈0.05）。结论葡萄糖耐量正常与异常孕妇的75gOGTT各时段血糖值比值存在明显差异,这种差异可能与胰岛素抵抗有关。Objective To investigate the value of the ratio of various data in 75 g oral glucose tolerance test （OGTT） between pregnant women with normal and abnormal glucose tolerance. Methods 75 g OGTT were performed among 1 909 pregnant women during 24 - 36 gestational week. All the pregnant women were divided into three groups according to the diagnostic criteria of American Diabetes Association： gestational diabetes mellitus （ GDM ） group （ n = 60 ） , gestational impaired glucose tolerance （GIGT） group （ n = 156 ） and normal glucose tolerance （NGT） group （n = 1 693）. The ratios of various data （PG3h/FPG, PG2h/FPG, PGlh/FPG, PG3h/PGlh, PG2h/PGlh and PG3h/PG2h） in 75 g OGTT were calculated and analysed. Results There was no significant difference in all the ratios between GDM group and GIGT group and in PG3h/FPG among the three groups （P 〉 0.05）. The ratios of PG2h/FPG and PGlh/ FPG in the GDM group and GIGT group were significantly higher than those in the NGT group（ P 〈 0.05）. While the ratios of PG3h/PGlh, PG2h/PGlh and PG3h/PG2h in the GDM group and GIGT group were significantly lower than those in the NGT group （P 〈 0.05）. Conclusion There were significantly differences in the ratios of various data in 75 g OGTT between pregnant women with normal and abnormal glucose tolerance, which may be associated with insulin resistance.
