未婚未育输卵管妊娠的危险因素分析

摘　　要：目的探讨与未婚未育输卵管妊娠发病相关的危险因素。方法收集2004年8月至2006年8月住院治疗的未婚未育输卵管妊娠患者91例(病例组),以同期在门诊就诊的未婚未育宫内早期妊娠者182例作为对照(对照组)。对调查表收集的资料进行单因素分析和非条件多元Logistic回归分析,探究危险因素。结果未婚未育输卵管妊娠发病的危险因素依次为:原因不明慢性盆腔疼痛、经期性交、多个性伴侣,其本身和(或)性伴侣为初中或以下文化程度、社会职业为无业、个体、服务员等。病例组上述情况的发生明显多于对照组。对照组性伴侣饮酒情况显著多于病例组,提示性伴侣饮酒可能为保护因素。结论未婚未育妇女的妇科疾病史、性生活状况、文化程度、社会职业以及性伴侣人口学特征等均与输卵管妊娠发病有关。对具有高危因素的人群加强保健检查和健康教育是十分必要的。Objective To explore the risk factors of fallopian tubal pregnancy in single and nonparous women. Methods Ninety-one single and nonparous cases who were hospitalized for fallopian tubal pregnancy were recruited from Aug 2004 to Aug 2006 and 182 single and nonparous cases with intrauterine early pregnancy from outpatient were served as controls. The data of questionaire were processed by univaxiate analysis and unconditional multivariate Logistic regression analysis, and the risk factors were detected. Results The pelvic pain with unknown origin, menstrual sexual intercourse, multiple sex partners, lower level of education （ below senior high school, herself and/or her sex partner） , unemployment, self-employment and job as a waitress were main high risk factors for single and nonparous women suffering from fallopian tubal pregnancy. While drinking was more commonly found in the sex partners of the control group than the case group, indicating a potential protective role of drinking of the sex partner. Conclusion The gynecological disease history, sexual life condition, educational background, employment condition and sex partner＇s demographic characteristics are associated with the fallopian tubal pregnancy in single and nonparous women. Therefore, it is necessary to strengthen the healthy examination and education among those with high risk factors.
