培哚普利对糖尿病合并高血压患者心功能和病死率的影响

摘　　要：目的 探讨培哚普利对糖尿病合并高血压患者心功能和病死率的影响。方法 糖尿病合并高血压患者左室舒张功能障碍121例，随机分为治疗组91例，对照组30例。两组均给予尼群地平10-20mg，3次/d;消心痛10mg，3次/d;肠溶阿斯匹林100mg，1次/d;氢氯噻嗪12.5mg，1次/d;治疗组加服培哚普利4mg，1次/d，6个月为一疗程，通过双盲方法，采用超声心动图进行对比观察。结果 反映左室形态的指标（LVID、LVMW），反应左室舒张功能指标（E峰、A峰、E/A比值、VTIE、VTIA、E-VTI/A-VTI），培哚普利治疗组明显改善，自身比较与对照组比较差异极显著性（P〈0.01）。临床疗效评定，治疗组总有效率93.4%，对照组总有效率53.3%，两组比较疗效有统计学意义（P〈0.01），治疗组明显优于对照组。结论 培哚普利不仅可改善左室舒张功能，逆转扩大的左心室，并明显降低死亡率。Objective To investigate the effects of perindopril on cardiac function and death reate of patients of diabetes with hypertension. Methods The two groups （ treatment group of 91 patients and the control group of 30 patients） were given the following drugs respectively. In the treatment group, perindopril 4-6 mg. d 1, isosorbide dinitrate 10mg. Tid aspilin 100mg. d-1, niqundiping 10mg q6h, hydrochlorothiazide 12.5 mg. d 1 were given to the patients, while in the control group, isosorbide dinitrate 10mg. Tid aspilin 100mg. d- 1, niqundiping 10-20mg q 6 h, hydrochlorothiazide 12.5 mg. d - 1were given to the patients for 6 months. Results With double-blind method, echocardiography was used to compare cardiac parameters before and after the ventricular dilatation（（VmaxE, VmaxA, E/A, VTIE, E-VTI/A-VTI） which were significantly improved in the treatment group. While there was no such obvious improvement in the control group （P 〈0.01）. The total effective rate of cardial function improvement was 93.4%, in the control group the rate was 53.3%. Fatalitiy rate was 1.1% and 10.0% in the treatment group and control group. Conclusion Perindopril could reverse the dilated left ventricle, improve left ventricular diastolie function and reduce fatality rate.
