摘  要:目的探讨傅立叶变换法无创测定高血压患者脑血流自动调节下限的有效性。方法招募符合入组标准的原发性高血压患者25例和健康志愿者35名为研究对象。用TCD、无创血压监测仪持续记录两组MCA的血流流速和桡动脉血压,用潮气末CO2监测仪持续监测呼出气体的CO2分压。用傅立叶变换法分别测得两组的临界关闭压和脑血流自动调节下限。结果高血压组的脑血流自动调节下限为(70±17)mmHg,较对照组[(60±11)mmHg]高约10mmHg,两组比较差异具有统计学意义(t=-2.645,P=0.012);而高血压组和对照组的临界关闭压差异无统计学意义。结论采用傅立叶变换法测定高血压患者脑血流自动调节下限是可行的,可望将来应用于高血压患者。Objective To investigate the efficacy of noninvasive estimating the lower limit of cerebral autoregulafion (LLCA) in patients with hypertension by Fourier transform method. Methods Twenty-five patients with essential hypertension and 35 healthy volunteers were recruited in the study. The blood flow velocity of middle cerebral artery and the blood pressure in radial artery in both hypertension group and control group were recorded continuously with transcranial Doppler and noninvasive blood pressure monitor. Partial pressure of carbon dioxide ( PCO2 ) of exhaled air was continuously monitored by an end-tidal CO2 monitor. The critical closing pressure (CCP) and LLCA were calculated respectively in both groups by Fourier transform method. Results The LLCA in the hypertension group was about 10 mm Hg higher than that in the control group, and there were significant difference between the two groups (t = -2. 645, P=0. 012). Conclusion It is feasible to estimate the LLCA in patients with hypertension by using Fourier transform method, and it is expected to be used selectively in the future in patients with hypertension.
