摘 要:[目的]为核因子-κB(NF-κB)介导病毒基因反式激活触发激素敏感型单纯性肾病(SRSNS)提供反面证据,并探讨糖皮质激素对NF-κB活性的抑制作用。[方法]采用电泳迁移率改变实验、RT-PCR和ELISA分别测定SRSNS、肾炎性肾病、继发性肾小球疾病、毛细支气管炎和正常儿童外周血单个核细胞(PBMCs)中NF-κB活性、呼吸道病毒基因和血浆病毒抗体;同时采用Westernblot和实时定量RT-PCR分别检测SRSNS和正常儿童IκBα蛋白质、IκBαmRNA表达水平。[结果]与SRSNS缓解期和其他组比较,SRSNS活动期组NF-κB活性明显增加。SRSNS活动期NF-κB活性与IκBα蛋白质水平呈直线负相关(r=-0.884,P﹤0.05)。SRSNS活动期复发和SRSNS缓解期患儿NF-κB活性低于SRSNS初发患儿,而IκBαmRNA和IκBα蛋白质高于后者。[结论]IκBα在SRSNS病毒基因反式激活中发挥重要作用,糖皮质激素通过诱导IκBα合成来抑制NF-κB活性。[Objoctive] To provide proof of viral transactivation of transcription mediated by NFκB in steroid responsive simple nephrotic syndrome (SRSNS) triggered by respiratory tract viruses, and to determine whether glucocorticoid can inhibit the activity of NF-KB. [Methods] SRSNS at active stage (including incipient stage and relapse stage) were examined, and were compared with SRSNS at remissive stage, nephritic nephrosis, secondary glomerular diseases, bronchiolitis and healthy children. Electro-mobility shift assays, RT-PCR and ELISA were used to detect the activity of NF-KB, the gene expression of respiratory tract viruses (including respiratory syncytial virus and itdluenza virus) in peripheral blood mononuclear cells (PBM- Ca) and the level of viral antibody in plasma, respectively. The protein level of I-kappa B alpha (IκBα), the mRNA level of IκBα were measured through western blot and ELISA in SRSNS at active stage, SRSNS at remissive stage and healthy children. [ Results ] Compared with SRSNS at remissive stage and other groups, the activity of NF-KB from SRSNS at active stage was higher statistically. A linear negative correlation between the activitv of NF-κB and the protein level of IκBα from SRSNS at active stage was observed (r = -0.884, P〈 0.05) . The activity of NF-κB from SRSNS at relapse stage and SRSNS at remissive stage were lower than that from SRSNS at incipient stage. However, the protein and gene levels of IκBα from SRSNS at relapse stage and SRSNS at remissive stage were much higher than those from SRSNS at incipient stage. [ Conclusions] IκBα may play an important role in viral transactivation of transcription mediated by NF-κB in SRSNS. Glucocorticoid-mediated repres- sion of the activity of NF-κB during viral transactivation of transcription involves inducting IκBα synthesis.
