播散性隐球菌病的临床诊断——附1例尸检病例报告并文献复习

摘　　要：目的探讨播散性隐球菌病的诊断与鉴别诊断。方法对1例播散性隐球菌病的尸体解剖病例进行病理学检查并文献复习。结果播散性隐球菌病是一种渗出性或肉芽肿性病变,可侵犯全身多个器官,临床表现多种多样,容易误诊或漏诊,预后差。结论播散性隐球菌病是一种少见病,临床表现无特异性,病理学检查、组织液涂片镜检和组织液培养发现隐球菌是诊断的关键,早期诊断有助于改善预后。Objective To explore the clinical diagnosis and differential diagnosis of disseminated cryptococcosis. Methods A case of autopsy with disseminated cryptococcosis was studied and discussed in reference to the literature. Results Disseminated cryptococcosis was characterized by exudative and granulomatous lesions. The bacteria might infect many organs with complex clinical features, and it is easy to be misdiagnosed, Its prognosis would be poor. Conclusion Disseminated cryptococcosis is rare and the clinical manifestion has no specificity. Pathological examination and repeated microscopic examination of interstitial fluid and interstitial fluid culture of cryptococcos are critical to make a definite diagnosis.Earlier diagnosis can improve the prognosis of disseminated cryptococcosis.
