摘  要:目的了解不同性别ST-T改变对诊断左心室肥大的临床意义。方法对符合心电图左心室肥大Sokolow-Lyon标准的67例男性、43例女性,根据左心室导联ST-T改变情况分成劳损组(29例)、早期劳损组(19例)和正常组(62例),观察其与超声心动描记术诊断左心室肥大(左心室后壁厚度>12mm)的符合情况。结果劳损组中超声诊断为左心室肥大者男性为94%,女性为50%(P<0.05)。早期劳损组中左心室肥大者男性为73%,女性为25%(P<0.05)。正常组中男女性患者左心室肥大比例差异无显著性意义(P>0.05)。结论QRS电压结合ST-T改变诊断左心室肥大的准确性高于单纯QRS电压标准,尤其是在男性。Objective To evaluate clinical significance of ST-T changes and gender differences in diagnosis of left ventricular hypertrophy. Methods 67 male and 43 female with left ventricular hypertrophy (LVH) on electrocardiogram (Sokolow-Lyon criteria) were classified into strain group(n=12),early strain group (n=19) and normal group (n=62) based on ST-T changes in left precordial leads. LVH was determined as left ventricular posterior wall thickness was 〉12mm on echocardiogram. Results LVH on echocardiogram was seen in 94% male and 50% female patients in strain group, in 73% male and 25% female patients in early strain group. There was significant difference of LVH prevalence between male and female in either of the two groups (P〈0.05), but not in normal group. Conclusion QRS voltage combined with ST-T changes may be a more accurate method than QRS voltage alone in diagnosis of LVH, especially in male.
