摘  要:目的观察胸腺肽α1(Tα1)对严重外科感染病人炎症因子及免疫功能的影响,探讨Tα1在外科感染治疗中的作用。方法将2005年2月至2006年5月南京军区南京总医院ICU收治的45例严重外科感染病人,随机分为用药组(24例)和对照组(21例),在控制感染的基础上用药组加用胸腺肽α1,并观察细胞因子水平的变化以及T细胞亚群CD3+、CD4+、CD8+及NK细胞百分率的变化。结果两组病人肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)水平均呈升高趋势,但对照组趋势更加明显,而白介素-10(IL-10)在用药组病人中呈升高趋势,治疗第4天用药组的CD3+、CD4+较用药前升高,明显高于对照组。结论Tα1可以调节严重感染病人肿瘤坏死因子(TNF-α)、白细胞介素6(IL-6)、白细胞介素10(IL-10)等细胞因子的水平,减轻炎症反应,改善病人的免疫功能。Objective To investigate the effectivity of thymosin α1 on immune function and cytokines in the abdominal sepsis patients,and discuss the therapeutic function of thymosin α1 in sepsis. Methods Forty-five sepsis patients admitted between February 2005 and May 2006 in the Research Institute of General Surgery of Nanjing General Hospital of Nanjing Military Command of PLA were randomly divided into the treatment group (24 cases) and control group (21 cases). Thymosin α1 was used in the treatment group on the basis of infection control. The concentration of cytokines( TNF-α,IL-6,IL-10) and the percent of CD3, CD4, NK cell were observed. Results The concentrations of TNF-α and IL-6 were increased significantly in both group. But it was more prominently in the control group. The concentration of IL-10 and percent of CD3, CD4, NK cell increased more significantly in thymosin α1 treatment group than in the control group. Conclusion Thymosin α1 can modulate the release of TNF-α, IL-6, IL-10, mitigate the inflammatory reaction and improve the immune function.
