摘 要:目的使用功能性磁共振的BOLD技术为基础来评价针刺技术对大脑皮层神经生理学的反应。方法对30位健康志愿者进行检查;其中对7位右利手志愿者进行针刺光明穴(GB37)、右侧阳陵泉穴(GB34)、左侧足三里穴(S36)和左侧伏兔穴(S32)的功能性磁共振检查;男性:6,女性:1,年龄:32-39岁;平均35.86岁。常规磁共振矢状位做结构像,功能磁共振检查使用EPI序列,完成实验计划:在一个扫描过程中,针刺分为以下几个步骤:1.先静态扫描30秒,2.然后在持续扫描的同时进针但不捻针222秒。3.在持续扫描过程中捻针15秒。4.持续扫描,留针不捻针,189秒时拔针。5.持续扫描至拔针后扫描180秒;同时使用工作站专业脑功能磁共振图像后处理软件。结果在全部的受试志愿者中,查后均有酸、麻、胀等得气现象,全部的受试者的大脑皮层均可在相应区域检测出明确的兴奋灶。结论使用功能性磁共振来探测针灸对脑皮层的反应是有一定意义的,对研究针灸穴位与中枢神经系统是否有相关性提供了有效的检测工具,对祖国中医针灸学方面的研究提供可信赖的方法。Purpose To evaluate the neurophysiological effects of acupuncture on brain with BOLD- based fMRI technique. Methods To research 30 healthy volunteers, including 7 volunteers with acupuncture of points Guangming (GB37), Yanglingquan (GB34), Zusanli (S36) and Futu(S32), located in the lateral aspect of lower leg. The data are acquired on GE 1.5 T Signa Horizon/Echo Speed scanner, with single shot EPI and 8 slices paralleled to AC-PC line. T2 * images were acquired with following parameters : flip angle 60, TR3000 ms , TE 40ms , FOV 25cm, Slth. 7mm, Gap 0.5mm, matrix 64 × 64, & 1 NEX. The procedure consists of 1.insertion of needle, 2. Twisting of the needle for 15 seconds and 3. taking out of the needle, with 220s time interval between 1 and 2 and 189s time interval between 2 and 3. Data sets of sequential images were analyzed with software package developed by R.W. Cox. Only pixels with a correlation coefficient of more than 0.7 were considered as significant and were used to create the functional map. Results 7 volunteers had definite activation in the brain cortex. They both had ‘De Qi' -phenomenon , a local tingling sensation which is essential for the effectiveness of acupuncture according to classic literature . 1. Three areas of activation in three lobes, frontal lobe, temporal lobe and occipital lobe were observed in 7 subjects. Although they are in the same lobe, but not exactly in the same location. 2. In additional activated foci were seen in the occipital lobe, insular and cerebellum. Conclusion The result shows a strong regional change in the BOLD-signal upon insertion of the acupuncture needle. There is considerable consistency and regional specificity of the observed effects.
